低密度脂蛋白胆固醇及相关因素与脑出血关系的Meta分析

【摘要】 目的:以Meta分析方法评价血清LDL-C水平及其它相关因素与脑出血的关系。方法:检索Pubmed、EMbase、Cochrane Library Datebase、CNKI、VIP、万方等数据库,以低密度脂蛋白和高血压和脑出血或LDL-C and hypertension and intracerebral hemorrhage为检索关键词,收集建库至2020年10月的文献,采用NOS质量评价法评价各研究的质量,以State 15.0软件合并统计量并进行敏感性检验,以egger法进行发表偏倚分析。结果:共纳入31篇病例对照研究文献,脑出血组和对照组各为9 759和11 477例。Meta分析结果表明,脑出血组血清LDL-C水平(2.70mM)较对照组(2.78mM)低,其差异有统计学意义(WMD,-0.50;95%CI:-0.74~-0.26)。脑出血组高血压患者比例、糖尿病患者比例、吸烟者及饮酒者比例均较对照组为高且差异有统计学意义,敏感性分析结果表明本文结果稳定性均较好,Egger法分析结果表明本文各指标均无发表偏倚。结论:脑出血组LDL-C水平较低,高血压患者、糖尿病患者、吸烟者及饮酒者比例较高,这些因素都可能和脑出血有关。

【Abstract】 Objective: To analysis the relationship between the intracerebral hemorrhage and the blood LDL level and some related factors by Meta analysis. Methods: Pubmed, medline, EMbase, Cochrane Library English Database and CNKI, VIP, WanFang Chinese Database were searched, and the key words were LDL-C AND hypertension AND intracerebral hemorrhage,The duration of search time was from the day of database establishment to October, 2020. The quality of every study was assessed according to the Newcastle-OttawaScale-(NOS). Various statistical analysises including statistical quantities combination, homogeneity assessment, sensitivity analysis, egger test used for publication bias analysis were performed with State 15.0software. Results: In our Meta analysis a total of 31 case-control studies were included, with a total of 9 759patients in intracerebral group and 11477 in control group. Meta-analysis results showed that the blood LDL level in intracerebral hemorrhage group(2.70 mM),is lower than that in the control group(2.78 mM)and the difference between of these two groups is statistically significant(WMD=-0.50;95%CI :-0.74--0.26). It is statistically significant higher in the ratio of the included hypertension patient, diabetes patient,smoker and drinker in the intracerebral hemorrhage group than in the control group. The result of sensitivity analysis showed that the stabilities of all the studies except BMI is good. It was proved with egger analysis that there is no publication bias in this analysis. Conclusions: Lower blood LDL level, the higher ratio of hypertension patient, diabetes patient, smoker and drinker can be related with intracerebral hemorrhage.
