急性心肌梗死患者血FT3水平与冠状动脉血流速度及钙化的相关性分析

【摘要】 目的 探讨急性心肌梗死患者血清游离三碘甲状腺原氨酸(FT3)水平与冠状动脉血流速度及钙化的相关性。方法选取2017年10月至2019年12月舟山医院诊断为急性心肌梗死且行急诊经皮冠状动脉介入(PCI)的患者283例。其中男249例,女34例。根据FT3水平将患者分为低FT3组(FT3:<4.1 pmol/L,123例)和正常FT3组(FT3:4.1~6.9 pmol/L,160例),比较两组患者性别、年龄、血生化、甲状腺功能等临床指标。根据患者冠状动脉造影结果有无钙化将患者分为钙化组(53例)和无钙化组(230例),比较两组患者的上述临床指标。结果 正常FT3组和低FT3组性别、年龄、血红蛋白、甘油三酯、高密度脂蛋白胆固醇、急诊PCI冠状动脉血流速度[以校正的TIMI帧数(CTFC)表示]及冠状动脉钙化差异均有统计学意义(均P<0.05)。钙化组和无钙化组比较,性别、年龄高、血红蛋白、FT3、总三碘甲状腺原氨酸(T3)差异均有统计学意义(均P<0.05)。线性回归分析显示,FT3是CTFC的独立影响因素,随着FT3升高,CTFC降低。多因素logistics回归分析显示,影响患者发生冠状动脉钙化的独立影响因素为性别和FT3(OR=2.446、0.372)。结论 急性心肌梗死患者FT3水平与冠状动脉血流速度和冠状动脉钙化之间存在相关性。

【Abstract】 Objective To investigate the correlation between serum free triiodothyronine(FT3) and coronary artery blood flow velocity and calcification in patients with acute myocardial infarction(AMI). Methods A total of 283 patients who were diagnosed with acute myocardial infarction and underwent emergency PCI in Zhoushan Hospital from October2017 to December 2019 were selected. There were 249 males and 34 females. According to the level of FT3, the patients were divided into low FT3group(FT3<4.1 pmol/L, n=123) and normal FT3group(FT3: 4.1-6.9 pmol/L, n=160). Blood biochemistry, thyroid function and other clinical indicators. The patients were divided into calcification group(n=53) and non-calcification group(n=230) according to their coronary angiography, and the above clinical indicators were compared between the two groups. Results There were significant differences in gender, age, hemoglobin, triglyceride, HDL-C,coronary blood flow velocity in emergency PCI(with TIMI flowmeter frame method, CTFC) and coronary calcification in the low FT3group(all P<0.05). There were significant differences in gender, age, hemoglobin, FT3, and total FT3between the calcification group and the non-calcification group(all P<0.05). Linear regression analysis showed that FT3was an independent factor affecting CTFC, and CTFC decreased as FT3increased. Multivariate logistic regression analysis showed that the independent influencing factors of coronary calcification in patients were gender and FT3(OR=2.446,0.372). Conclusion FT3is correlation with coronary flow velocity and calcification in patients with AMI.
