残余胆固醇对冠状动脉支架再狭窄影响的临床研究

【摘要】 目的 探讨残余胆固醇(Remnant-C)对冠状动脉支架再狭窄(ISR)的影响。方法 选择2015年6月至2021年12月在宁波市医疗中心李惠利医院心内科住院的冠状动脉支架植入术后的患者1 980例,均行冠状动脉造影术,术后1年根据冠状动脉ISR的诊断标准采用简单随机抽样法选取再狭窄组100例和未狭窄组100例,收集两组患者的临床资料及冠状动脉造影结果。采用单因素分析和多因素logistic回归分析冠状动脉ISR的危险因素。结果 再狭窄组的Remnant-C水平为(0.48±0.23)mmol/L,高于未狭窄组的(0.31±0.23)mmol/L(P<0.01),再狭窄组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[LP(a)]、载脂蛋白B(ApoB)、糖化血红蛋白(HbA1c)及SYNTAX评分均高于未狭窄组,差异均有统计学意义(均P<0.01),logistic回归分析结果提示Remnant-C和LP (a)是冠状动脉ISR的危险因素(均P<0.05)。结论 Remnant-C可为PCI术后患者的临床管理提供有价值的数据,避免ISR及其他心血管事件的发生,值得临床推广应用。

【Abstract】 Objective To explore the influence of remnant cholesterol on coronary in-stent restenosis. Methods A total of 1 980 patients underwent coronary stenting in the department of Cardiovascular Medicine, Ningbo Medical Center LI HUILI Hospital from June 2015 to December 2021, and coronary angiography was performed for these patients.One year after PCI, 100 cases in the restenosis group and 100 cases in the unrestenosis group were selected by simple random sampling according to the diagnostic criteria of coronary in-stent restenosis, the clinical data and coronary angiography results of the two groups were collected and compared. Univariate analysis and multivariate logistic regression were used to analyze risk factors for coronary in-stent restenosis. Results Remnant cholesterol was significantly higher in restenosis group[(0.48±0.23) mmol/L] than in no-restenosis group[(0.31±0.23) mmol/L](P<0.01).TC, LDL-C, LP(a), ApoB, HbA1C and SYNTAX scores were significantly higher in restenosis group than in no-restenosis group(all P<0.01). Logistic regression analysis showed that remnant cholesterol and LP( a) were risk factors for coronary in-stent restenosis( all P < 0. 05). Conclusion Remnant cholesterol is related to the occurrence of in-stent restenosis after coronary stent implantation.
