术后患者自控镇痛泵报警状况及其原因分析
Setting off the alarm of patient-controlled postoperative analgesic pump: an investigation and analysis of the causes
【摘要】 目的 了解患者术后使用自控镇痛泵报警现况,并分析原因,提出预防和改进措施。方法 回顾性调查2020年3月—6月1 774例术后应用无线自控镇痛泵患者(其中静脉镇痛1 348例,占76.0%,硬膜外镇痛321例,占18.1%,其他105例,占5.9%),在应用无线自控镇痛泵过程出现报警情况,并分析原因。结果 1 774例患者在应用自控镇痛泵过程中发生7 502例次报警,报警主要原因为离开服务区和管道堵塞,分别为3 487例次(占46.5%)和3 015例次(占40.2%)。其中数据缺失是离开服务区报警的主要原因,为3 483例次(占99.9%),数据缺失与管路打折为管道堵塞报警的主要原因,分别为1 752例次与1 251例次(分别占58.1%与41.5%)。结论 应根据自控镇痛泵报警情况,采取针对性的对策,降低术后患者自控镇痛泵报警发生率,以提高患者镇痛效果与满意度,促进患者康复。
【Abstract】 Objective To investigate the alarming of patient-controlled analgesic pumps in postoperative patients and analyse the causes of alarm setting off, hence to improve the measures in prevention of alarming. Methods Between March and June, 2020, 1,774patients with postoperative analgesia were enrolled in the retrospective investigation, including 1,348(76.0%) cases of PCIA, 321(18.09%)cases of continuous epidural analgesia, accounting and 105(5.9%) cases of other analgesic methods. All of the analgesic pumps were wireless PCA pumps. The times and reasons of alarm setting off were analysed. Results A total of 7,502 times of alarms were raised among the 1,774 patients. The alarms was mainly set off by the beyond the range of wireless signal and the pipe blockage, at 3,487 times(46.5%)and 3,015 times(40.2%), respectively. Data missing due to loss of wireless signal was the main reason for alarming in 3,483 cases(99.9%).Data missing and tangled pipeline were the main reasons for alarming from pipe blockage, in 1,752(58.1%) and 1,251(41.5%) cases,respectively. Conclusions Precise and reliable measures should be taken to reduce the alarm rate of the analgesic pumps and to improve the effect of analgesia and the satisfaction of patients.
