CT与MRI诊断膝关节韧带损伤的影像学表现与价值

【摘要】 目的:对比计算机断层扫描(CT)与磁共振成像(MRI)在膝关节韧带损伤中的诊断价值及影像学表现。方法:选择甘肃省人民医院2020年1月-2021年5月收治的膝关节韧带损伤患者76例进行对比研究,患者均接受CT检查、MRI检查,将关节镜检查结果为作为诊断的“金标准”,计算CT、MRI诊断膝关节韧带损伤准确率,并观察影像学表现差异。结果:MRI在膝关节韧带损伤诊断中的准确率高于CT,差异有统计学意义(P<0.05)。膝关节韧带损伤CT影像学表现为韧带挛缩、增宽,交叉韧带可见断裂,图像显示低密度。膝关节韧带损伤MRI影像学表现:急性期T1低信号,T2高信号。慢性期T1信号长于T2,韧带可见增粗、扭曲,断端边缘不清,断痕明显,信号异常。前交叉韧带增宽、变粗,走形区信号增高;韧带连续性中断,可见游离影,飘带样改变;韧带股骨附着点连续性中断,可见异常增高信号;韧带断端挛缩,走形区信号增高。结论:膝关节韧带损伤临床诊断中应用CT和MRI均可获得较高的诊断价值,MRI诊断准确率高于CT,且获得的影像学表现内容更加丰富、完整、清晰,为诊断提供了有效参考依据,可以作为首选诊断方式。

【Abstract】 Objective: To compare the diagnostic value and imaging findings of computed tomography(CT) and magneticresonance imaging(MRI) in knee ligament injury. Methods: From January 2020 to May 2021, 76 patients with knee ligament injuryin Gansu Provincial People’s Hospital were selected for comparative study. All 76 patients were examined by CT and MRI, andarthroscopy result served as the "gold standard" for diagnosis to calculate the accuracy of CT and MRI in the diagnosis of kneeligament injury, and to observe the imaging findings. Results: The accuracy of MRI in the diagnosis of knee ligament injury washigher than that of CT, and the difference was statistically significant(P<0.05). CT imaging findings of knee ligament injuryincluded contracture and widening of the ligament, visible fracture of the cruciate ligament, and low density. MRI imaging findingsof knee ligament injury included low T1 signal and high T2 signal in acute stage. The signal length of T1 in chronic stage was longerthan that of T2, the ligament was thickened and twisted, the edge of broken end was unclear, the broken mark was obvious, and thesignal was abnormal. The anterior cruciate ligament was widened and thickened, and the signal in the shape area increased. Theligament continuity was interrupted, the dissociation shadow and ribbon-like change were visible; The continuity of the attachmentpoint of the ligament femur was interrupted, and abnormally high signal was observed. Contractures at the broken end of theligament and increased signal in the shape area were observed. Conclusion: The application of CT and MRI in the clinicaldiagnosis of knee ligament injury can obtain higher diagnostic value, the MRI diagnosis accuracy is higher than CT, and theobtained imaging findings are more abundant, complete and clear, providing effective reference for diagnosis, can be used as thepreferred diagnosis method.
