肾结石融合影像超声导航下经皮肾镜取石术后一期清石情况的影响因素分析

【摘要】 目的 探讨肾结石融合影像超声导航下经皮肾镜取石术(PCNL)后一期清石情况的影响因素。方法 回顾性分析150例在融合影像超声导航下行PCNL的肾结石患者的临床资料,分析PCNL术后一期清石情况的影响因素。结果 单因素分析结果显示,铸型结石情况、手术时间可能与肾结石患者融合影像超声导航下PCNL术后一期清石有关(均P<0.05)。多因素Logistic回归分析结果显示,铸型结石和手术时间≥120 min是融合影像超声导航下PCNL术后不能实现一期清石的独立影响因素(均P<0.05)。结论 PCNL术前应提前了解铸型结石等结石形态,做好术中个体化方案,同时注意控制手术时间并尽量清石,以提高一期清石率。

【Abstract】 Objective To investigate the influencing factors of one-stage calculus clearance after percutaneous nephrolithotomy(PCNL) guided by renal calculus fusion image ultrasound. Methods The clinical data of 150 renal calculus patients who underwent PCNL guided by fusion image ultrasound was retrospectively reviewed to analyze the influencing factors of one-stage calculus clearance after PCNL. Results The results of univariate analysis showed that the staghorn calculus and operation time might be related to one-stage calculus clearance in patients with renal calculus after PCNL guided by fusion image ultrasound(all P<0.05). Multivariate logistic regression analysis showed that staghorn calculus and operation time ≥120 minutes were the independent influencing factors for the failure of one-stage calculus clearance after PCNL guided by fusion image ultrasound(all P<0.05). Conclusion Before PCNL, it is necessary to know such calculus shape as staghorn calculus in advance, make individualized intraoperative plan, pay attention to control the operation time and try to remove calculus, so as to improve the one-stage calculus removal rate.
