14~16岁人工耳蜗植入儿童听力语言康复效果研究

【摘要】 目的 探讨人工耳蜗植入对14~16岁听障儿童听力语言康复效果。方法 选择2017年3月~2019年10月我院收治的人工耳蜗植入患儿130例,对比分析患儿人工耳蜗植入前后助听听阈,植入后6个月、12个月重建听阈、听觉功能及语言功能。结果 人工耳蜗植入后6个月、12个月对应频率(在500、1000、2000和4000 Hz不同频率的啭音信号中)的重建听阈值比植入前助听听阈值显著降低,植入后12个月重建听阈值<植入后6个月<植入前(P<0.05);人工耳蜗植入后6个月、12个月听障儿童的声母、韵母、短句识别率及听觉行为分级(CAP)均比植入前高,植入后12个月听觉能力>6个月>植入前(P<0.05);人工耳蜗植入后6个月、12个月听障儿童的听话识图、主题对话模块与同龄健听儿童相当的言语年龄更高,言语可懂度分级(SIR)更高,植入后12个月语言能力>6个月>植入前(P<0.05)。结论 人工耳蜗植入能明显改善大龄听障儿童的听觉能力和语言能力。

【Abstract】 Objective To investigate the effect of cochlear implant on hearing and speech rehabilitation of hearingimpaired children aged 14-16 years. Methods 130 children with hearing impairment who received cochlear implant in our hospital from March 2017 to October 2019 were selected. The hearing thresholds before cochlear implantation, and reconstructed hearing thresholds, auditory function and language function of children at 6 months and 12 months after cochlear implantation were compared and analyzed. Results(1) Aided hearing threshold: 6 months and 12 months after implantation,the aided hearing threshold at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz significantly decreased compared with those before implantation. Aided hearing thresholds: 12 months after implantation < reconstructed hearing threshold at 6 months after implantation < hearing threshold before implantation(P<0.05).(2) Hearing ability: Hearing-impaired children had higher recognition rates of initials, vowels, short sentences, and categories of auditory performance(CAP) grades at 6 months and 12 months after implantation than before implantation. Hearing ability at 12 months after implantation > hearing ability at 6months after implantation > hearing ability before implantation(P<0.05).(3) Language Ability: At 6 months and 12 months after implantation, the hearing-impaired children had a higher speech age and a higher speech intelligibility rating(SIR)than normal children of the same age. Language ability at 12 months after implantation > language ability at 6 months after implantation > language ability before implantation(P<0.05). Conclusion Cochlear implantation can obviously improve the hearing ability and language ability of children with hearing impairment.
