【摘要】 目的:探讨人工膝关节置换与关节冲洗封闭法对治疗重度骨性关节炎老年患者的效果。方法:回顾性分析我院78例重度骨性关节炎老年患者临床资料(2016年1月至2019年1月),其中接受关节冲洗封闭治疗患者定为对照组(n=37),接受人工膝关节置换术患者定为研究组(n=41),统计两组治疗前后疼痛程度(VAS)、膝关节功能(Lysholm、HSS)、治疗优良率和并发症发生率。结果:(1)治疗后1周、1个月两组VAS分值较治疗前降低(P<0.05),且对照组低于研究组(P<0.05);(2)治疗后3个月、6个月两组Lysholm及HSS分值较治疗前增高(P<0.05),且研究组高于对照组(P<0.05);(3)研究组治疗效果优于对照组(P<0.05);(4)研究组并发症发生率(16.22%)与对照组(5.41%)间无显著差异(P>0.05)。结论:人工膝关节置换术后疼痛控制作用不及关节冲洗封闭治疗，术后并发症发生率与关节冲洗封闭治疗无明显差异，但其可显著改善关节功能，提高疾病治疗效果。更多还原

【Abstract】 Objective: To investigate the effect of artificial knee joint replacement and joint flushing closure on the treatment of elderly patients with severe osteoarthritis. Methods: The clinical data of 78 elderly patients with severe osteoarthritis(January 2016-January 2019) were retrospective analyzed, among whom patients treated with joint flushing closure were designated as controls(n=37), patients treated with artificial knee replacement(n=41) as treatment group, and the degree of pain(VAS), knee function(Lysholm, HSS), excellent treatment rate and complication rate were calculated. Results:(1) One week and 1 month after treatment, the VAS score decreased(P<0.05) and the control group was lower than the study group(P<0.05);(2) the Lysholm and HSS score increased 3 months and 6 months after treatment(P<0.05) and they were higher in the study group than in the control group(P<0.05);(3) better treatment effect was achieved in the study group than in the control group(P<0.05);(4) There was no significant difference between the incidence of complications(16.22%) and the control group(5.41%)(P>0.05). Conclusion: The pain control effect of artificial knee joint replacement is not as good as that of joint irrigation closure treatment, and the incidence of postoperative complications is not significantly different from that of joint flushing closure treatment, but it can significantly improve joint function and disease treatment effect.更多还原
