经外周置入中心静脉导管术前术后不同定位标志影像资料可比性分析及临床应用
Comparability analysis of preoperative and postoperative imaging data of PICC with different positioning markers
【摘要】 目的 对比经外周置入中心静脉导管(PICC)后胸部X线定位片与胸部CT定位片上导管头端与不同定位标志的位置差异,分析PICC术前术后不同定位标志影像资料可比性。方法 选择河南科技大学第一附属医院2020年1月至2021年6月PICC置管患者149例,所有病例符合病例选择标准,置管后常规摄胸部站立后前位X线定位片监测导管头端位置,收集置管后1个月内患者胸部CT检查资料,测量胸部X线定位片、CT定位片上PICC导管头端至邻近胸椎、肺门角、气管隆突距离。结果 胸部X线定位片PICC导管头端至邻近的胸椎椎体上缘(或下缘)的平均距离为(-2.46±0.62)mm,CT定位片上平均距离为(-0.95±0.66)mm,二者对比差异有统计学意义(t=-2.94,P=0.004);胸部X线定位片上PICC导管头端至肺门角平均距离为(-31.0±19.2)mm,CT定位片上平均距离为(-34.2±16.8)mm,二者对比差异有统计学意义(t=-2.85,P=0.005);胸部X线定位片PICC导管头端至气管隆突的平均距离为(-51.3±19.3)mm,CT定位片上平均距离为(-51.8±17.4)mm,二者对比差异无统计学意义(t=-0.43,P=0.67)。结论 相对于胸椎、肺门角,以气管隆突作为定位标志,PICC术前术后影像资料具有更好的一致性和可比性。
【Abstract】 Objective To compare the position differences of catheter head end and different positioning marks on chest X-ray positioning film and chest CT localization film after PICC catheterization,and to analyze the comparability of imaging data before and after PICC with different positioning marks. Methods A total of 149patients with PICC in No.1 Affiliated Hospital of Henan University of Science and Technology from January 2020 to June 2021 were enrolled. All patients met the criteria of case selection. After catheterization, the position of the catheter head was monitored by routine chest X-ray positioning. The chest CT examination data of patients within 1month after catheterization were collected. The distance of PICC catheter head to adjacent thoracic vertebrae, hilar angle and tracheal protuberance on chest X-ray positioning film and CT localization film were measured. Results The average distance from the head of PICC catheter to the upper(or lower) edge of the adjacent thoracic vertebral body on chest X-ray positioning film was(-2.46 ±0.62) mm, and that on CT positioning film was(-0.95 ±0.66) mm.There was significant difference between the two groups(t=-2.94, P=0.004). The average distance from the head of PICC catheter to the hilar angle on chest X-ray positioning film was(-31.0±19.2) mm, and that on CT positioning film was(-34.2±16.8) mm. There was significant difference between the two groups(t=-2.85, P=0.005). The average distance from the head of PICC catheter to tracheal protuberance on chest X-ray positioning film was(-51.3 ±19.3)mm, and that on CT film was(-51.8±17.4) mm.There was no significant difference between the two groups(t=-0.43,P=0.67). Conclusion Compared with thoracic vertebra and hilum angle, the image data before and after PICC have better consistency and comparability with tracheal protuberance as the positioning mark.
