高分辨MR血管壁成像对颅内血栓性动脉瘤形成的相关因素分析

【摘要】 目的 分析高分辨磁共振血管壁成像(HRMR-VWI)对颅内血栓性动脉瘤的相关特征。方法 对61例行CT血管成像(CTA)、数字减影血管造影(DSA)发现颅内动脉瘤的患者行HRMR-VWI序列平扫及增强检查,评估血栓的存在与动脉瘤特征的关系及病因分析。结果 61例颅内动脉瘤患者中,有血栓者(血栓组)23例,无血栓者(无血栓组)38例。2组中高血压、空腹高血糖水平存在显著差异(P<0.05),血栓组共有24个动脉瘤,无血栓组共有45个动脉瘤。2组在动脉瘤发生的部位存在统计学意义(P<0.05)。瘤壁强化、动脉瘤部位、高血压及总胆固醇(TC)升高对动脉瘤血栓的形成具有显著影响(P<0.05)。结论 采用HRMR-VWI可以对颅内血栓性动脉瘤的形态学特征进行准确的评估。

【Abstract】 Objective To investigate the raletive characteristics of intracranial thrombotic aneurysms by high-resolution magnetic resonance vessel wall imaging(HRMR-VWI).Methods 61 patients who underwent computed tomography angiography(CTA)and digital subtraction angiography(DSA) for the detection of intracranial aneurysms underwent HRMR-VWI sequence plain and enhanced examinations to evaluate the relationship between the presence of thrombus and aneurysm characteristics and analysis the etiology.Results Among 61 patients with intracranial aneurysms,23 cases had thrombosis(thrombotic group) and 38 cases had no thrombosis(non-thrombotic group).There were significant differences in the levels of hypertension and fasting hyperglycemia between the two groups(P<0.05).There were 24 aneurysms in the thrombotic group and 45 aneurysms in the non-thrombotic group.There was a statistically significant difference between the two groups in the location where the aneurysms occurred(P <0.05).The enhancement of the aneurysm wall,aneurysm location,hypertension,and elevated total cholesterol(TC) contributed significantly to the formation of aneurysm thrombosis(P<0.05).Conclusion The use of HRMR-VWI in patients with intracranial thrombotic aneurysms allows accurate assessment of their morphological features.
