【摘要】 目的:探究超声联合血清CA₁₂₅检测对结核性输卵管炎的诊断临床价值。方法:选取接受治疗的80例疑似结核性输卵管炎患者为研究对象，分别对其开展经阴道超声检查及血清学检测，以病理学检查结果为金标准，分别评估经阴道超声、血清CA₁₂₅以及阴道超声联合CA₁₂₅检测对结核性输卵管炎的诊断价值，并分别绘制ROC曲线计算曲线下面积（AUC）。结果:（1）经阴道超声对结核性输卵管炎诊断一致性为61.25%,灵敏度为58.82%,特异度为75.00%,阳性预测值为93.02%,阴性预测值为24.32%;（2）血清CA₁₂₅对结核性输卵管炎诊断一致性为60.00%,灵敏度为63.24%,特异度为41.67%,阳性预测值为86.00%,阴性预测值为16.67%;（3）经阴道超声联合血清CA₁₂₅对结核性输卵管炎诊断一致性为96.25%,灵敏度为97.06%,特异度为91.67%,阳性预测值为98.51%,阴性预测值为84.62%;（4）超声对结核性输卵管炎诊断AUC为0.6500（P=0.0011）,血清CA₁₂₅对结核性输卵管炎诊断AUC为0.6125（P=0.0140）,联合检测对结核性输卵管炎诊断AUC为0.6563（P=0.0006）。结论:经阴道超声与血清CA₁₂₅对结核性输卵管炎都具有较好的诊断效能，但联合检测能够明显提高诊断的一致性、灵敏度和特异度，建议临床上可将上述两种方式联合起来用于结核性输卵管炎的初筛中。更多还原

【Abstract】 Objective:To explore the clinical value of ultrasound combined with serum CA₁₂₅ in the diagnosis of tuberculous salpingitis.Methods: Eighty patients with suspected tuberculous salpingitis were selected as the research object, and transvaginal ultrasound and serological examination were carried out respectively.Taking pathological examination results as the gold standard, the diagnostic value of transvaginal ultrasound, serum CA₁₂₅ and transvaginal ultrasound combined with CA₁₂₅ in tuberculous salpingitis was evaluated respectively, and ROC curves were drawn to calculate the area under the curve（AUC）.Results:The consistency of transvaginal ultrasound in the diagnosis of tuberculous salpingitis was 61.25%,the sensitivity was 58.82%,the specificity was 75.00%,the positive predictive value was 93.02%,and the negative predictive value was 24.32%.The consistency of serum CA₁₂₅ in the diagnosis of tuberculous salpingitis was 60.00%,the sensitivity was 63.24%,the specificity was 41.67%,the positive predictive value was 86.00%,the negative predictive value was 16.67%.The consistency of transvaginal ultrasound combined with serum CA₁₂₅ in the diagnosis of tuberculous salpingitis was 96.25%,the sensitivity was 97.06%,the specificity was 91.67%,the positive predictive value was 98.51%,the negative predictive value was 84.62%.The AUC for ultrasound diagnosis of tuberculous salpingitis was 0.6500（P=0.0011）,for serum CA₁₂₅ was 0.6125（P=0.0140）,and for combined detection was 0.6563（P=0.0006）.Conclusion:Both transvaginal ultrasound and serum CA₁₂₅ have good diagnostic efficacy for tuberculous salpingitis, but combined detection can obviously improve the consistency, sensitivity and specificity of diagnosis.It is suggested that the above two methods can be combined for the primary screening of tuberculous salpingitis clinically.更多还原
