【摘要】 目的:调查成人下肢骨折术后恐动症的发生现状，分析其影响因素，并进行路径分析。方法:抽取在山西省某三级甲等医院手术治疗的211例下肢创伤性骨折病人作为研究对象，采用一般情况调查表、疼痛数字评分表（NRS）、恐动症Tampa评分表（TSK）、匹兹堡睡眠质量指数（PSQI）及创伤后应激障碍自评量表（PCL-C）对其进行调查。结果:211例病人中140例发生恐动症，发生率为66.4%。Logistic回归分析结果显示，疼痛程度、主观睡眠质量、睡眠效率及再体验症状是下肢创伤性骨折病人恐动症发生的影响因素（P<0.05）。路径分析显示，主观睡眠质量可直接正向预测恐动症，同时主观睡眠质量与睡眠效率可以疼痛程度为中介变量影响恐动症的发生。结论:护理人员应密切关注病人的心理状态，采取针对性措施提高病人睡眠质量，减轻病人疼痛，以减少病人恐惧感，促进病人早期康复。更多还原

【Abstract】 Objective:To investigate the status quo of adult patients with kinetophobia after lower extremity fracture,analyze the influencing factors,and conduct the path analysis. Methods:Totally 211 patients with lower extremity traumatic fracture,who were surgically treated in a tertiary grade A hospital in Shanxi province were selected as the research object. The subjects were investigated by general condition questionnaire,Pain Number Rating Scale(NRS),Tampa Scale of kinesiophobia(TSK)Pittsburgh Sleep Quality Index(PSQI)and Posttraumatic Stress Disorder Checklist-Civilian Version(PCL-C). Results:Out of 211 patients,140 patients had kinesiophobia,the incidence rate was 66. 4%. Logistic regression analysis showed that pain degree,subjective sleep quality,sleep efficiency and reexperience symptoms were the iinfluencing factors of kinesiophobia in patients with lower extremity traumatic fracture(P<0. 05). Path analysis showed that subjective sleep quality was a direct and positive predictor of kinetophobia,and subjective sleep quality and sleep efficiency were mediating variables affecting the occurrence of kinetophobia. Conclusion:In clinical nursing work,we should pay close attention to the patient’s psychological state,take measures to improve the patient’s sleep quality,relieve the patient’s pain,reduce the patient’s fear,in order to promote the patient’s early recovery.更多还原
