市级医院不同病情时间消耗指数的差异及相关因素分析
Differences and related factors of time consumption index of different illnesses in a municipal hospital
【摘要】 目的:探讨市级医院不同病情时间消耗指数的差异和相关影响因素,为当地医院的管理和决策提供参考依据。方法:收集广东省卫生健康委员会统计信息网络直报系统某三甲医院的疾病诊断相关分组结果及2020年1月1日—12月31日86 680例出院患者的病案首页数据。比较不同病例分型、不同手术级别、是否抢救和是否入住重症监护室(intensive care unit,ICU)患者的时间消耗指数。以时间消耗指数为因变量,选取差异有统计学意义的指标为自变量进行二分类Logistic回归分析。结果:不同病例分型、不同手术级别、是否抢救和是否入住ICU的患者时间消耗指数的差异均有统计学意义(P值均<0.001)。从时间消耗指数是否大于1 (全省平均水平)的可能性来看,OR值随着病情的加重而升高,其中病例分型为D型患者的时间消耗指数大于1的可能性是A型的1.708倍。手术患者的时间消耗指数大于1的可能性大于无手术操作的患者,其中二级手术患者的时间消耗指数大于1的可能性是无手术操作的1.300倍。需要抢救的患者时间消耗指数大于1的可能性是不需要抢救的1.353倍。入住ICU的患者时间消耗指数大于1的可能性是不需要入住ICU的1.186倍。结论:不同病情患者的时间消耗指数不具有同质性,病例分型、手术级别、是否抢救和是否入住ICU可能是影响时间消耗指数的相关因素。
【Abstract】 Objective: To investigate the difference and related influencing factors of the time consumption index of different illnesses in a municipal hospital, so as to provide a reference for the management and decision-making of the local hospital. Methods: The disease diagnosis-related grouping results of a tertiary hospital in the Guangdong Provincial Health Commission.s Statistical Information Network Direct Reporting System and the front page data of 86 680 discharged patients from January 1 to December 31, 2020 were collected. The time consumption index of patients with different case types, different surgical grades, whether they were rescued or not, and whether they were admitted to the intensive care unit(ICU) was compared. The time consumption index was used as the dependent variable, and the indicators with statistically significant differences were selected as independent variables for binary logistic regression analysis. Results: There were statistically significant differences in the time consumption index of patients with different case types, different surgical grades, whether they were rescued or not, and whether they were admitted to the ICU(all P<0.001). From the perspective of the possibility of whether the time consumption index was greater than 1(the average level of the province), the OR value increased with the aggravation of the disease, and the probability of the time consumption index of type D patients being greater than 1 was 1.708 times that of type A. Surgery patients were more likely to have a time consumption index greater than 1 than those without surgery, and patients with secondary surgery were 1.300times more likely to have a time consumption index greater than 1. Patients requiring rescue were 1.353 times more likely to have a time consumption index greater than 1. Patients admitted to the ICU were 1.186 times more likely to have a time consumption index greater than 1 than those who did not require ICU admission. Conclusion:The time consumption index of patients with different conditions is not homogenous. Case type, surgical grade,whether rescue and ICU admission may be the relevant factors affecting the time consumption index.
