基于倾向评分匹配探究体重指数与骨龄提前的关系

【摘要】 目的:基于倾向评分匹配探究体重指数与骨龄提前的关系。方法:回顾性分析2016年9月—2020年12月在汕头大学医学院第一附属医院全科医学分院儿童生长发育专科门诊的404例儿童和青少年的临床资料,其中男性171例,女性233例,年龄3~17岁,平均年龄(8.09±2.83)岁。将研究对象分为体重正常组(357例)及超重肥胖组(47例)。利用倾向性评分均衡两组的混杂因素后,采用Logistic回归模型分析骨龄提前的影响因素。结果:倾向评分匹配前,除了父亲身高及促甲状腺激素水平外,其他因素在两组间分布不平衡(P值均<0.05)。匹配后,所有协变量在两组间达到均衡(P值均>0.05)。倾向评分匹配前,超重肥胖组骨龄提前的发生风险是体重正常组的7.235倍(OR=7.235,95%CI:3.770~13.883,P<0.001)。匹配后,超重肥胖组骨龄提前的发生风险是体重正常组的4.789倍(OR=4.789,95%CI:2.079~11.033,P<0.001)。结论:超重肥胖可能是骨龄提前的独立危险因素,控制体重增长有利于预防骨龄提前的发生,提高儿童和青少年的成年终身高。

【Abstract】 Objective: To investigate the relationship between body mass index and bone age advance based on propensity score matching. Methods: The clinical data of 404 children and adolescents from September 2016 to December 2020 in the Department of Child Growth and Development of the General Medicine Branch of the First Affiliated Hospital of Shantou University Medical College were retrospectively analyzed, including 171 males and233 females, aged 3-17 years old, mean age(8.09±2.83) years old. The subjects were divided into normal weight group(357 cases) and overweight and obesity group(47 cases). After equalizing the confounding factors between the two groups by propensity score, logistic regression model was used to analyze the influencing factors of advanced bone age. Results: Before propensity score matching, except for father.s height and thyroid-stimulating hormone level, other factors were unevenly distributed between the two groups(all P<0.05). After matching, all covariates were balanced between the two groups(all P>0.05). Before propensity score matching, the risk of occurrence of bone age advancement in the overweight and obese group was 7.235 times higher than that in the normal weight group(OR=7.235, 95%CI: 3.770-13.883, P<0.001). After matching, the risk of occurrence of bone age advancement in the overweight and obese group was 4.789 times higher than that in the normal weight group(OR=4.789, 95%CI: 2.079-11.033, P<0.001). Conclusion: Overweight and obesity may be an independent risk factor for early bone age. Controlling weight gain is beneficial to prevent the occurrence of bone age advancement and improve adult height in children and adolescents.
