肝硬化上消化道大出血患者行普萘洛尔辅助治疗的效果及对再出血发生率的影响
The effect of propranolol adjuvant therapy and its influence on the incidence of rebleeding in patients with liver cirrhosis with upper gastrointestinal bleeding
【摘要】 目的:研究肝硬化上消化道大出血患者应用普萘洛尔辅助治疗的疗效。方法:选取2019年6月~2022年2月收治的108例肝硬化上消化道大出血患者,按随机数字表法分组,对照组(54例)在常规治疗基础上加用奥曲肽治疗,治疗组(54例)在对照组基础上加用普萘洛尔辅助治疗,对比两组疗效、炎性因子、门脾静脉血流量、再出血率。结果:治疗组96.30%总有效率高于对照组79.63%(P <0.05);治疗前1dCRP、IL-6及IL-10水平比较,两组差异无统计学意义(P> 0.05);与对照组比较,治疗组治疗后5dCRP、IL-6水平更低,IL-10水平更高(P <0.05);治疗前1d门、脾静脉血流量比较,两组差异无统计学意义(P> 0.05);治疗组治疗后5d门、脾静脉血流量低于对照组(P <0.05);治疗组3.70%再出血率低于对照组20.37%(P <0.05)。结论:肝硬化上消化道大出血患者应用普萘洛尔辅助治疗,可促进疗效提高,降低再出血率。
【Abstract】 Objective: To study the efficacy of propranolol adjuvant therapy in patients with liver cirrhosis with upper gastrointestinal bleeding. Methods: A total of 108 patients with liver cirrhosis with upper gastrointestinal hemorrhage who were treated from June 2019 to February 2022 were selected and divided into groups according to the random number table method. The control group(54 cases) was treated with octreotide on the basis of conventional treatment, and the treatment group(54 cases) was treated with octreotide. Example) On the basis of the control group,propranolol was added as adjuvant therapy, and the efficacy, inflammatory factors, portal and splenic vein blood flow,and rebleeding rate were compared between the two groups. Results: The total effective rate of the treatment group was 96.30% higher than that of the control group, which was 79.63%(P<0.05). There was no significant difference in the levels of CRP, IL-6 and IL-10 between the two groups at 1d before treatment(P>0.05). In comparison, the levels of CRP and IL-6 in the treatment group were lower at 5d after treatment, and the level of IL-10 was higher(P<0.05); there was no significant difference in the blood flow of the portal and splenic veins between the two groups at 1d before treatment(P>0.05). 5d after treatment, the blood flow of portal and splenic vein in the treatment group was lower than that in the control group(P<0.05); the rebleeding rate in the treatment group was 3.70% lower than that in the control group(P<0.05),20.37%. Conclusion: Adjuvant propranolol therapy can improve the curative effect and reduce the rate of rebleeding in patients with upper gastrointestinal bleeding in liver cirrhosis.
