【摘要】 目的：探讨粉碎性指骨骨折患者内固定治疗后手指功能恢复的影响因素。方法：回顾性收集2015年2月～2019年5月某院收治的68例粉碎性指骨骨折患者临床资料，患者均行内固定治疗，记录患者手指功能恢复情况，并记录患者一般资料，经单因素与多因素分析明确粉碎性指骨骨折患者内固定治疗后手指功能恢复不良的影响因素。结果：68例粉碎性指骨骨折患者中手指功能恢复不良12例，恢复良好56例，手指功能恢复良好组与手指功能恢复不良组患者性别、骨折类型、优势手情况对比，差异无统计学意义(P>0.05);手指功能恢复不良组患者年龄大于手指功能恢复良好组，合并关节内骨折、合并软组织损伤占比高于手指功能恢复良好组，伤后至手术时间长于手指功能恢复良好组，差异有统计学意义(P<0.05);经Logistic回归分析结果显示，年龄较大、合并关节内骨折、合并软组织损伤、伤后至手术时间长是粉碎性指骨骨折患者内固定术治疗后手指功能恢复不良的影响因素(OR>1,P<0.05)。结论：粉碎性指骨骨折患者内固定治疗后手指功能恢复情况与患者年龄、是否合并关节内骨折或软组织损伤、伤后至手术时间关系密切。更多还原

【Abstract】 Objective:To investigate the influencing factors of finger function recovery in patients with comminuted phalangeal fracture after internal fixation.Methods: The clinical data of 68 patients with comminuted phalangeal fracture treated in a hospital from February 2015 to may 2019 were retrospectively collected. All patients were treated with internal fixation. The finger function recovery and the general information of the patients were recorded. The influencing factors of poor finger function recovery after internal fixation in patients with comminuted phalangeal fracture were identified by the univariate and multivariate analysis.Results: In 68 cases of comminuted phalangeal fractures, 12 cases had poor finger function recovery and 56 cases had good recovery. There was no statistically significant difference in gender, fracture type and dominant hand between the good finger function recovery group and poor finger function recovery group(P>0.05);The age of patients in poor finger function recovery group was older than that in good finger function recovery group, the proportion of patients complicated with intra-articular fracture and soft tissue injury in poor finger function recovery group was higher than that in good finger function recovery group, and the time from injury to operation was longer than that in good finger function recovery group, the difference was statistically significant(P<0.05);Logistic regression analysis showed that older age, complicated with intra-articular fracture, complicated with soft tissue injury, and long time from injury to operation were the influencing factors of poor recovery of finger function after internal fixation in patients with comminuted phalangeal fracture(OR>1,P<0.05). Conclusion: The recovery of finger function in patients with comminuted phalangeal fracture after internal fixation is closely related to the age of patients, whether complicated with intra-articular fracture or soft tissue injury, and the time from injury to operation.更多还原
