冷循环微波消融联合TACE对肝癌患者肝功能指标及甲胎蛋白水平的影响

【摘要】 目的:探讨冷循环微波消融联合肝动脉栓塞化疗(TACE)在肝癌患者中的治疗效果。方法:选取2020年1月~2021年1月某院收治的96例肝癌患者,按随机数字表法分为对照组和观察组各48例,对照组给予TACE,观察组加用冷循环微波消融,连续治疗3个月。对比两组临床疗效、肝功能指标、甲胎蛋白(AFP)水平与肿瘤直径、不良反应。结果:观察组治疗总有效率较对照组高,差异有统计学意义(P<0.05);治疗前,两组总胆红素(TBIL)、谷草转氨酶(AST)、丙氨酸转氨酶(ALT)、AFP、肿瘤直径相比,差异无统计学意义(P>0.05);治疗后,观察组TBIL、AST、ALT和AFP、肿瘤直径较对照组低,差异有统计学意义(P<0.05);两组不良反应相当,差异无统计学意义(P>0.05)。结论:冷循环微波消融与TACE能够降低肝癌患者AFP水平,促使肿瘤坏死,有利于改善肝功能,且无严重不良反应,安全可行。

【Abstract】 Objective: To investigate the therapeutic effect of cold-circulation microwave ablation combined with hepatic artery embolization chemotherapy(TACE) in patients with liver cancer. Methods: A total of 96 liver cancer patients admitted to a hospital from January 2020 to January 2021 were selected and divided into two groups according to the random number table method, with 48 cases in each group. The control group was given TACE, and the observation group was treated with cold-circulation microwave ablation for 3 consecutive months. The clinical efficacy, liver function indexes, alpha-fetoprotein(AFP) level, tumor diameter, and adverse reactions were compared between the two groups. Results: The total effective rate of treatment in the observation group was higher than that in the control group, and the difference was statistically significant(P<0.05). Before treatment, there was no statistically significant difference in total bilirubin(TBIL), aspartate aminotransferase(AST), alanine aminotransferase(ALT), AFP and tumor diameter between the two groups(P>0.05). After treatment, the TBIL, AST, ALT and AFP, and tumor diameter in the observation group were lower than those in the control group, and the difference was statistically significant(P<0.05). The adverse reactions of the two groups were similar, and the difference was not statistically significant(P>0.05). Conclusion: Cold-circulation microwave ablation combined with TACE can reduce AFP level in patients with liver cancer, promote tumor necrosis, and improve liver function without serious adverse reactions, which is safe and feasible.
