MRI与CT在膝关节损伤诊断中的临床应用效果对照分析

【摘要】 目的 对比磁共振成像(magnetic resonance imaging, MRI)与电子计算机断层扫描(computed tomography, CT)在膝关节损伤诊断中的临床应用效果。方法 回顾性分析2021年1—12月新泰市人民医院98例疑似膝关节损伤就诊患者的临床资料,该组患者均行CT检查与MRI检查,将膝关节镜手术结果作为金标准,分析比较CT与MRI对膝关节损伤的诊断结果,诊断的敏感度、特异度、阳性预测值、阴性预测值、准确率,以及CT与MRI对膝关节损伤的检查时间。结果 该组疑似膝关节损伤患者确诊阳性73例(74.49%),阴性25例(25.51%)。其中CT检查阳性69例(70.41%),阴性29例(29.59%)。MRI检查阳性72例(73.47%),阴性26例(26.53%)。MRI诊断膝关节损伤的敏感度97.26%、特异度96.00%、阴性预测值92.31%与CT的87.67%、80.00%、68.97%对比,差异无统计学意义(χ~2=3.273、1.500、1.125,P>0.05),阳性预测值98.61%、准确率96.94%较CT的88.89%、85.71%高,差异有统计学意义(χ~2=4.000、5.882,P<0.05)。MRI的膝关节损伤检查时间(23.65±3.02)min较CT(10.02±2.45)min长,差异有统计学意义(t=-233.546,P<0.001)。结论 相较于CT,MRI对于膝关节损伤的诊断准确率更高,为临床诊疗工作提供了可靠的参考信息。

【Abstract】 Objective To compare the clinical effect of magnetic resonance imaging(MRI) and computed tomography(CT) in the diagnosis of knee injury. Methods The clinical data of 98 patients with suspected knee injury in Xintai People’s Hospital from January to December 2021were retrospectively analyzed. All patients in the group underwent CT examination and MRI examination, and the results of knee arthroscopy were regarded as the gold standard. The diagnostic results, sensitivity, specificity, positive predictive value, negative predictive value, accuracy, as well as the examination time of CT and MRI for knee injury were analyzed and compared. Results There were 73 positive cases(74.49%) and 25 negative cases(25.51%) in the group of patients with suspected joint injury. Among them, CT examination was positive in 69cases(70.41%) and negative in 29 cases(29.59%). MRI examination was positive in 72 cases(73.47%) and negative in 26 cases(26.53%).The sensitivity, specificity, and negative predictive value of MRI in diagnosing knee injury were 97.26%, 96.00%, and 92.31% compared with CT’s 87.67%, 80.00%, and 68.97%, and the difference was not statistically significant(χ~2=3.273, 1.500, 1.125, P> 0.05), the positive predictive value was 98.61%, and the accuracy rate was 96.94% higher than that of CT’s 88.89% and 85.71%, and the difference was statistically significant(χ~2=4.000, 5.882, P<0.05). The examination time of MRI for knee injury was(23.65±3.02) min longer than that of CT(10.02±2.45)min, and the difference was statistically significant(t=-233.546, P<0.001). Conclusion Compared with CT, MRI is more accurate in diagnosing knee injury, providing reliable reference information for clinical diagnosis and treatment, and is suitable for clinical promotion.
