慢性肾功能衰竭患者周围神经电生理特点
Electrophysiological characteristics of peripheral nerves in patients with chronic renal failure
【摘要】 目的:分析慢性肾功能衰竭患者周围神经中运动神经及感觉神经的电生理特点。方法:选择2020年11月至2021年5月在潍坊市坊子区人民医院血液透析中心诊治的尿毒症患者32例作为尿毒症组,健康志愿者17例作为对照组,检测尿毒症患者和对照组上下肢神经的运动神经传导速度、末端潜伏期及波幅,感觉神经传导速度、峰潜伏期及波幅。结果:尿毒症组1例胫后神经、腓总神经运动神经未引出;与对照组比较,尿毒症组患者各运动神经末端潜伏期明显延长(P<0.05),运动神经传导速度降低(P<0.01),除腓总神经复合肌肉动作电位(CMAP)波幅降低外[(6.2±2.9) mV比(8.8±3.0) mV,t=4.129,P<0.001],其余运动神经CMAP波幅差异无统计学意义(P>0.05)。感觉神经中1例腓浅神经未引出,4例胫后神经、足底内侧神经、足底外侧神经未引出;与对照组比较,尿毒症组患者感觉神经峰潜伏期延长,感觉神经传导速度及动作电位波幅降低(P<0.05)。结论:慢性肾功能衰竭患者易并发周围神经病变,电生理改变中感觉神经变化较运动神经敏感。
【Abstract】 Objective: To analyze the electrophysiological characteristics of motor nerve and sensory nerve in peripheral nerve of patients with chronic renal failure. Methods: Thirty-two patients with uremia treated in the hemodialysis center of Fangzi District People’s Hospital of Weifang City from November 2020 to May 2021 were enrolled as the uremia group and 17 healthy volunteers as the control group. The motor nerve conduction velocity,distal latency and amplitude, sensory nerve conduction velocity, peak latency and amplitude of the upper and lower limb nerves were examined in all subjects. Results: The motor nerves of posterior tibial nerve and common peroneal nerve were not elicited in one case of uremia group. Compared with the control group, the distal latency of motor nerve in uremia group was significantly prolonged(P<0.05), the motor nerve conduction velocity was decreased(P<0.01),and the amplitude of complex muscle action potential(CMAP) of common peroneal nerve was decreased(P<0.001).Among the sensory nerves, the superficial peroneal nerve was not elicited in one case, and the posterior tibial nerve,medial plantar nerve and lateral plantar nerve were not elicited in 4 cases. Compared with the control group, the peak latency of sensory nerve in uremia group was prolonged, and the conduction velocity and amplitude of action potential were decreased(P<0.05). Conclusion: Patients with chronic renal failure are easily complicated with peripheral neuropathy. Sensory nerve changes are more sensitive than motor nerve changes.
