帕金森病周围神经损害肌电图特点

【摘要】 目的:探讨帕金森病周围神经损害的肌电图特点。方法:选取2019年8月至2020年12月福建省立医院神经内科住院的帕金森病患者25例,采用丹迪Keypoint4肌电/诱发电位仪进行检查,分析25例帕金森病患者周围神经的肌电图特点。结果:(1)被检神经感觉纤维损害的比例高于运动纤维(63.2%比31.0%,P<0.05);(2)被检神经波幅异常率高于传导速度异常率(83.1%比34.2%,P<0.05);(3)被检神经潜伏期、传导速度及波幅双侧比较差异无统计学意义(P>0.05);(4)上肢神经异常率低于下肢神经异常率(36.0%比76.0%,P<0.05);(5) F反应异常主要表现为潜伏期延长;(6)四肢皮肤交感反应(SSR)上下肢差异无统计学意义(P>0.05);(7)神经传导异常率和SSR异常率比较,差异无统计学意义(P>0.05);(8)被检肌肉未见明显异常。结论:帕金森病周围神经损害是一种以感觉纤维轴索损害为主并可伴或不伴有一定程度自主神经损害的周围神经病。

【Abstract】 Objective: To explore the electromyogram(EMG) characteristics of peripheral nerve injuries in Parkinson’s disease(PD). Methods: Twenty-five PD patients in Department of Neurology, Fujian Provincial Hospital from August 2019 to December 2020 were enrolled in the study. The EMG characteristics of peripheral nerves in 25 patients with PD were analyzed. Results:(1) The proportion of sensory fiber injuries was higher than that of motor fiber(63.2% vs.31.0%, P<0.05).(2) The abnormal rate of amplitude of detected nerve was higher than that of conduction velocity(83.1%vs. 34.2%, P<0.05).(3) There was no significant difference in latency, conduction velocity and amplitude between the two sides(P>0.05).(4) The abnormal rate of upper limb nerve was lower than that of lower limb nerve(36.0% vs. 76.0%,P<0.05).(5) Abnormal F response appeared mainly in terms of prolonged latency.(6) There was no significant differences in the upper and lower limbs of the limb skin sympathetic response(SSR).(7) There was no significant difference for the abnormal rate between nerve conduction and SSR(P>0.05).(8) No obvious abnormality was found in the examined muscles.Conclusion: Peripheral nerve injuries in Parkinson’s disease is a kind of peripheral neuropathy with sensory fiber axonal injuries as the main factor and with or without autonomic nerve injuries to some extent.
