核素骨显像在识别骨质疏松椎体压缩性骨折黑线信号中的价值分析
Value Analysis of Radionuclide Bone Imaging in Identifying Black Line Signal of Osteoporotic Vertebral Compression fracture
【摘要】 目的 探讨核素骨显像在识别骨质疏松椎体压缩性骨折(OVCF)黑色线性信号中的价值与作用。方法 回顾性分析2018年9月1日至2021年7月1日我院48例OVCF患者X片、QCT、MRI和核素骨显像病例资料。根据MRI上信号不同表现分为黑色线性信号组(A组20例)和非黑色线性信号组(B组28例),比较两组患者一般资料、MRI黑色线性信号和核素骨显像浓聚形态及特征之间的相关性。结果 两组患者共涉及新鲜和陈旧性骨折椎体共56个,其中MRI诊断出新鲜骨折椎体43个,骨显像诊断出45个,结合患者临床和影像学表现最终确认责任椎体42个,对MRI和骨显像诊断结果采取Kappa检验(Kappa>0.75),表明两者诊断一致性较高。两组患者年龄、性别、骨密度值差异无统计学意义(P>0.05)。对比核素骨显像上造影剂汇聚形态及浓度与MRI上黑色线性信号的表现之间未见明显相关性(P>0.05)。结论 骨显像与MRI在诊断新鲜骨质疏松椎体压缩性骨折上具有高度一致性。MRI黑色线信号的作为一种可以早期预测OVCF患者预后的危险因素,骨显像在发现伴MRI黑色线性信号表现的OVCF患者时未见明显特异性。
【Abstract】 Objective To investigate the value and role of radionuclide bone imaging in identifying the black linear signal of osteoporotic vertebral compression fracture(OVCF). Methods From September 1,2018 to July 1,2021,retrospective analysis of 48 cases of OVCF patients hospitalized in our hospital was performed. All patients underwent X-ray, QCT,MRI and radionuclide bone imaging after admission. According to different manifestations of signals on MRI,they were divided into black linear signal group( Group A,n = 20) and non-black linear signal group( Group B,n = 28). After that,correlation in general information,black linear signal on MRI,morphology and characteristics of radionuclide bone imaging in these two groups was compared. Results 56 fresh and old vertebral bodies were involved in two groups. Among them,43 fresh vertebral bodies were diagnosed by MRI,and 45 were diagnosed by bone imaging. In addition,42 responsible vertebral bodies were finally confirmed based on clinical and imaging manifestations of patients. Kappa test( Kappa > 0. 75) was performed on MRI and bone imaging diagnosis results,indicating that diagnosis consistency between two groups was high. There was no significant difference in age,gender or BMD between two groups( P > 0. 05).Baseline data for two groups of patients was similar and comparable. There was no significant correlation between convergent morphology and concentration of the contrast media on radionuclide bone imaging and black linear signal on MRI( P > 0. 05). Conclusion Bone imaging and MRI have high agreement in the diagnosis of fresh osteoporotic vertebral compression fractures. As a risk factor for early prognosis of OVCF patients with MRI black line signal,bone imaging shows no obvious specificity in detecting OVCF patients with MRI black line signal.
