事件相关电位 N400在新诊断特发性或隐源性青年癫痫患者认知功能障碍研究中的应用

【摘要】 目的 探讨事件相关电位N400在评价新诊断特发性或隐源性青年癫痫患者在早期认知功能损害中的作用。方法 收集2012年6月至2019年3月新诊断特发性或隐源性青年癫痫患者23例作为癫痫组,以健康志愿者20例为对照组,详细记录癫痫病史并长程脑电图监测,两组均行简易精神状态检查(MMSE)量表评分、Moca量表评分、瑞文测验-联合型(IQ分),以排除临床已存在认知功能的癫痫患者;采用中文正常语句结尾关键词语义匹配与非匹配的模式对癫痫组及正常对照组进行事件相关电位N400检测,分析对比两组之间N400潜伏时间及波幅变化。结果 癫痫组患者事件相关电位N400的潜伏时间较正常组延长(P<0.05),波幅两组比较差异无统计学意义(P>0.05)。结论 事件相关电位N400可以为作评价癫痫患者早期的脑功能损害的电生理证据之一。

【Abstract】 Objective To study event-related potential N400 in evaluating early cognitive impairment of young patients with newly diagnosed idiopathic or cryptogenic epilepsy. Methods From June 2012 to March 2019,patients were collected. 23 patients with newly diagnosed idiopathic or cryptogenic young epilepsy as epilepsy group and 20 healthy volunteers as control group.Medical history of epilepsy group was recorded in detail and all patients were examined by electroencephalogram. Simple mental state examination(MMSE) scale score, Montreal Cognitive assessment(Moca) scale score and Raven’s progressive Matrices were used to exclude epileptic patients with cognitive function, then using Chinese normal sentence ending key word meaning matching and non-matching mode to detect event-related potential N400 in epilepsy group and normal control group. N400 latent time and amplitude were analyzed and compared.Results Latency of N400 of event-related potentials in epileptic group was longer than that in normal group(P<0.05). Amplitude was no obvious difference between two groups(P>0.05). Conclusion Event-related potential N400 could be used as one of the electrophysiological evidence to evaluate early brain function impairment in epileptic patients.
