lncRNA THRIL和miR-300在新生儿败血症患者中的临床意义

【摘要】 目的 探究长链非编码RNA-肿瘤坏死因子相关异种核蛋白L(lncRNA THRIL)和微小RNA-300(miR-300)在新生儿败血症患中的表达水平及临床意义。方法 选择2016年6月至2021年9月青岛市市立医院收治的132例新生儿败血症患儿为试验组,同期住院的77例非败血症一般感染新生儿为对照组,采用受试者工作特征(ROC)曲线及曲线下面积(AUC)评估lncRNA THRTL、miR-300对新生儿败血症的早期诊断及预后预测价值;采用Spearman相关性分析检测lncRNA THRTL、miR-300与白细胞计数(WBC)、降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)的相关性。结果 试验组lncRNA THRIL、miR-300表达量均高于对照组,差异有统计学意义(t值分别为-8.283、-7.596,P<0.001);lncRNA THRIL、miR-300、PCT早期联合诊断败血症患儿AUC最高,为0.975;lncRNA THRIL、miR-300与PCT、CRP、IL-6、TNF-α均呈正相关关系(r值介于0.269~0.441之间,P<0.05);lncRNA THRIL和miR-300联合评估败血症患儿预后的AUC最高,为0.867。结论 lncRNA THRIL和miR-300在新生儿败血症患儿血清中高表达,并且对新生儿败血症的早期诊断和预后有一定的预测价值。

【Abstract】 Objective To investigate the expression level and clinical significance of long non-coding RNA-tumor necrosis factor-related and heterogeneous nuclear ribonucleoprotein L-related immunoregulatory(lncRNA THRIL) and microRNA-300(miR-300) in patients with neonatal sepsis.Methods A total of 132 patients with neonatal sepsis admitted to Qingdao Municipal Hospital from June 2016 to September 2021 were selected as the experimental group, and 77 patients with non-sepsis general infection were selected as the control group.Receiver operating characteristic(ROC) curve and area under curve(AUC) were used to evaluate the early diagnosis and prognostic value of lncRNA THRTL and miR-300 for neonatal sepsis.Spearman correlation analysis was used to detect the correlations of lncRNA THRTL and miR-300 with white blood cell count(WBC),procalcitonin(PCT),C-reactive protein(CRP),interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α).Results The expression levels of lncRNA THRIL and miR-300 in the experimental group were higher than those in the control group, and the differences were statistically significant(t=-8.283 and-7.596,respectively, P<0.001).The AUC of early combined diagnosis of lncRNA THRIL,miR-300 and PCT was the highest, which was 0.975.lncRNA THRIL and miR-300 were positively correlated with PCT,CRP,IL-6 and TNF-α(r=0.269-0.441,respectively, P<0.05).lncRNA THRIL combined with miR-300 had the highest AUC of 0.867 for the prognosis of patients with neonatal sepsis.Conclusion lncRNA THRIL and miR-300 are highly expressed in the serum of patient with neonatal sepsis, and have certain value for the evaluation of early diagnosis and prognosis of neonatal sepsis.
