数字化导板在解除磨牙锁牙合中的设计与应用
Design and application of digital orthodontic miniscrew guides in the treatment of molar scissor bite
【摘要】 目的 基于3shape扫描和锥形束CT影像,设计并3D打印解除磨牙锁牙合的腭侧种植钉导板,评价其稳定性和临床疗效。方法 选择正畸治疗磨牙锁牙合患者20例,随机分成2组,每组10例。实验组进行数字化设计3D打印金属种植钉腭侧导板植入种植钉固位,以预设在导板上的牵引钩给颊倾磨牙加力。对照组以传统方式腭侧植入微种植钉加力,评价安全稳定性和临床疗效。结果 3D打印金属导板可实现双侧磨牙锁牙合的治疗;锁牙合解除实验组平均时间为(3.2±0.67)个月,对照组为(4.1±0.32)个月,实验组明显优于对照组(P<0.05);2组垂直向移动无明显差异,垂直移动主要发生在治疗早期腭尖明显压低;2组水平向移动主要发生在第2和第3个月,腭尖显著舌向移动,实验组优于对照组,且在第2个月时差异有统计学意义(P<0.05)。实验组20颗种植钉在治疗过程中无松动脱落,对照组脱落3颗在后期补种。结论 数字化腭侧种植钉导板在解除磨牙锁牙合中具有良好的安全性、稳定性和临床疗效。
【Abstract】 Objective Based on 3 Shape scanning and cone beam CT images, the palatal orthodontic miniscrew guides to treat the molar scissor bite was designed and printed in 3 D,and the stability and clinical efficacy were evaluated.Methods A total of 20 orthodontic patients with molar scissor bite were collected and randomly divided into two groups, each group has 10 patients.In the experimental group, the digital designed and 3 D printed metal miniscrew palatal guides fixed with miniscrew, and the traction hook presetted on the palatal guide was used to force the buccal inclined molars.In the control group, the traditional palatal miniscrew were implanted in the palatal side in the buccal inclined molars.The stability and clinical efficacy were evaluated between the two groups.Results The average time of scissor bite release was(3.2±0.67)months in the experimental group and(4.1±0.32) months in the control group.The experimental group was significantly better than that in the control group(P<0.05).There were no significant difference in vertical movement between the two groups.The vertical movement mainly occurred in the early stage of treatment, and the palatal cusp was obviously depressed.The horizontal movement of the two groups mainly occurred in the second and third months, with significant palatal cusp lingual movement, the experimental group was better than the control group, and there was statistical significance in the second month(P<0.05).20 implants nails in the experimental group did not loose during the treatment, while 3 implants were dropped and replanted in the control group.Conclusion Digital palatal miniscrew guide has good safety, stability and clinical efficacy in the treatment of molar scissor bite.
