应用颞浅动脉-大脑中动脉搭桥术治疗闭塞性脑血管疾病疗效与安全性meta分析

【摘要】 目的系统评价颞浅动脉-大脑中动脉搭桥术(superficial temporal artery-middle cerebral artery, STA-MCA)治疗闭塞性脑血管疾病的疗效与安全性。方法计算机检索PubMed、Embase、The Cochrane Library、中国知网及万方数据库,搜集自建库起至2021年3月所有关于STA-MCA治疗闭塞性脑血管疾病(cerebrovascular disease)的随机对照试验(randomized controlled clinical trials, RCTs)。由2位研究者共筛选出939篇文献,最终纳入8篇文献,总病例数为2 265例,其中试验组1 138例,对照组1 127例,使用Review Manager5.4软件进行研究分析。结果 STA-MCA手术组较药物治疗组治疗有效性高(OR=0.67,95%CI:0.47~0.96,P=0.03)。结论对于颅内闭塞性脑血管疾病应用STA-MCA搭桥手术是一种有效的方法,但是该结论仍然需要更多的优质研究加以论证。

【Abstract】 Objective To evaluate the efficacy and safety of superficial temporal artery-middle cerebral artery(STA-MCA)in the treatment of occlusive cerebrovascular disease.Methods PubMed, Embase, The Cochrane Library, CNKI and Wanfang databases were searched for randomized controlled clinical trials(RCTs)of STA-MCA in the treatment of occlusive cerebrovascular diseases from inception to March 2021.A total of 939 articles were screened by two researchers, and 8 articles were finally included, with a total of 2265 cases, including 1 138 cases in the experimental group and 1127 cases in the control group. Review Manager 5.4 software was used for research and analysis.Results The efficacy of STA-MCA operation group was higher than that of drug treatment group(OR=0.67,95% CI:0.47-0.96,P=0.03).Conclusion STA-MCA bypass surgery is an effective method for intracranial occlusive cerebrovascular diseases, but this conclusion still needs more high-quality studies to demonstrate.
