【摘要】 目的：探讨甲状腺钙化类型及相关临床病理特征在甲状腺结节性质鉴别诊断中的意义。方法：回顾性分析286例患者的术前临床资料，将超声钙化分为无钙化、微钙化、粗大钙化及混合钙化，以术后病理结果为金标准，探究甲状腺钙化及临床特征与甲状腺结节的关系。结果：甲状腺结节良、恶性组间年龄、体质指数、有无钙化、结节直径差异均有统计学意义（<0.05），其中良、恶性组中年龄≥45岁与<45岁、肥胖-正常组间、不同钙化类型均存在统计学差异（<0.05）。Logistic回归分析结果显示调整后BMI、钙化及TTD是甲状腺癌的危险因素（<0.05），但年龄与甲状腺癌无相关性（>0.05）。而不同钙化类型与PTC的总直径、多灶性及中央区淋巴结转移无统计学意义。结论：不同钙化类型、肥胖与甲状腺乳头状癌发生显著相关，临床诊疗中应高度重视。更多还原

【Abstract】 Objective: To investigate the significance of thyroid calcification types and related clinicopathological features in the differential diagnosis of thyroid nodules.Methods:The preoperative clinical data of 286 Patients were analyzed retrospectively.Ultrasonic calcification was divided into no calcification, microcalcification, coarse calcification and mixed calcification. According to the postoperative pathological results as the gold standard,to explore the relationship between thyroid calcification, clinical features and thyroid nodules.Results:There were significant differences in age,body mass index(BMI),calcification and nodule diameter between benign and malignant thyroid nodules( <0.05).There were significant differences between benign and malignant groups( <0.05).There were significant differences in patients older than 45 years old and younger than 45 years old, obesity-normal group and different types of calcification between benign and malignant groups( <0.05). Binary Logistic regression analysis showed that adjusted BMI, calcification types and TTD were risk factors for thyroid cancer, but there was no correlation between age and thyroid cancer( >0.05).There was no statistical significance between different types of calcification and PTC diameter, multiple foci and lymph node metastasis in central region.Conclusion:Different types of calcification and obesity are significantly related to the occurrence of papillary thyroid carcinoma, which should be Paid more attention to in clinical diagnosis and treatment.更多还原
