结肠癌微环境中免疫细胞浸润模式及其预后评估
The infiltration pattern of immune cells in colon cancer microenvironment and its prognostic significance
【摘要】 目的 探索结肠癌微环境中免疫细胞的浸润模式及其预后评估价值。方法 利用CIBERSORT软件包从癌症基因组图谱TCGA(The Cancer Genome Atlas)数据库、GEO(Gene Expression Omnibus)数据库共筛选38例正常结肠组织样本及316例结肠癌组织样本,基于标准化后的基因表达谱对22种浸润免疫细胞进行提取和量化,分析免疫细胞在正常组织和癌组织中的浸润差异。利用多因素COX回归构建列线图。结果 与正常结肠组织相比,活化的CD4~+记忆性T细胞、M0巨噬细胞、M1巨噬细胞在结肠癌组织中浸润显著增多,幼稚B细胞、浆细胞、单核细胞及未活化的肥大细胞显著减少。高水平活化的肥大细胞及调节性T细胞浸润与肿瘤转移和分期进展密切相关(P<0.05),低水平活化的CD4~+记忆性T细胞和滤泡辅助性T细胞与肿瘤淋巴结播散及分期进展相关(P<0.05)。基于差异浸润的免疫细胞构建的列线图预测1年、3年、5年生存率的ROC曲线下面积(area under the curve,AUC)分别为0.837、0.763和0.741,该模型具有良好预测效能。结论 结肠癌患者中免疫细胞的浸润差异可能在疾病进展中具有重要作用,其预后模型可能为结肠癌患者的生存评估提供理论和数据支持。
【Abstract】 To explore the infiltration pattern of immune cells infiltrating in colon cancer microenvironment and its prognostic significance, total of 38 normal colon tissues samples and 316 cancer tissues samples were collected from the Cancer Genome Atlas(TCGA) database and Gene Expression Omnibus(GEO) database. The proportion of22 types of immune cells in normal tissues and cancer tissues were deconvolved by CIBERSORT algorithm.Multivariate COX regression was used to construct the nomogram model. Data showed that the infiltration of activated CD4~+memory T cells, M0 macrophages and M1 macrophages were higher in colon cancer, while na?ve B cells, plasma cells, monocytes and resting mast cells were lower in colon cancer. Moreover, high proportions of activated mast cells and regulatory T cells were closely associated with tumor metastasis and staging progression(P<0.05), while low proportions of activated T cells CD4~+memory and follicular helper T cells were associated with lymph node dissemination and tumor stages(P<0.05). The area under the curve(AUC) of ROC were 0.837, 0.763,and 0.741, respectively, indicating that the nomogramhad good predictive ability. In addition, colon cancerpatients further confirmed the predictive value of themodel based on the risk score, and the results showedthat colon cancer patients in the high-risk group had apoor prognosis. In conclusion, the infiltration difference of infiltrating immune cells is critical in the development and progression of colon cancer patients, and its prognosticmodel probably provides support for the progression assessment and survival prediction of colon cancer.
