【摘要】 目的 研究血肿CT值在脑出血微创手术中的应用价值。方法 回顾性分析66例脑出血行微创穿刺手术治疗的患者的临床资料，统计血肿CT值、血肿体积、术前血肿扩大情况、术中抽除血肿量、血肿清除率以及术后再出血情况，统计分析血肿CT值与血肿扩大、血肿清除率及预后的相关性。结果 血肿CT值与血肿扩大及血肿清除率负相关，即随平均CT值的增高呈下降趋势(P<0.01),使用尿激酶再出血风险与平均CT值呈负相关(P<0.01),中CT值(60～80HU)患者比高CT值及低CT值患者预后好(P<0.05)。结论 (1)低CT值患者血肿清除率高，但尿激酶治疗后并发症同样高，预后不理想；(2)高CT值患者血肿清除率低，但尿激酶治疗后并发症低，预后尚可；(3)中CT值患者血肿清除率中间水平，尿激酶治疗后并发症低，预后好。更多还原

【Abstract】 Objective To study the value of application of CT value during the minimally invasive surgery for intracerebral hemorrhage.Methods The clinical data of 66 cases of cerebral hemorrhage with minimally invasive surgery were retrospectively analyzed.The CT value of hematoma, hematoma volume, the enlargement of preoperative hematoma, intraoperative hematoma extraction volume, hematoma clearance rate and postoperative rebleeding were statistically analyzed.The correlation among the CT value of hematoma and hematoma enlargement, hematoma clearance rate and prognosis were statistically analyzed.Results The CT value of the hematoma was negatively correlated with the enlargement of the hematoma and the clearance rate of the hematoma.That means with the increase of the average CT value, the enlargement of the hematoma and the clearance rate of the hematoma decreased.There was a negative correlation among them(P<0.01).The risk of rebleeding after using urokinase was negatively correlated with the average CT value(P<0.01).The patients with medium CT value(60～80 HU) had better prognosis than patients with high CT value and low CT value(P<0.05).Conclusions(1)Low CT value patients had high hematoma clearance rate, but the complications after urokinase treatment were also high.The prognosis was not ideal.(2)Though the hematoma clearance rate of patients with high CT value was low, the complications after urokinase treatment were low, and the prognosis was fair.(3)The hematoma clearance rate of patients with medium CT value was in the middle level, and the complications after urokinase treatment were low.The prognosis was good.更多还原
