膝关节骨关节炎手术治疗效果的影响因素

【摘要】 目的 探讨膝关节骨关节炎手术治疗效果的影响因素,以期制定针对性干预措施。方法 选择2019年1月-2019年12月我院收治的80例膝关节骨关节炎患者作为研究对象,所有患者均行人工关节置换术治疗,术后随访3个月,采用美国膝关节协会(AKS)评分评估两组膝关节综合改善情况。统计所有患者基线资料,包括游离体、体重指数(BMI)、软骨损伤、韧带损伤、半月板损伤、术后感染、术后下肢深静脉血栓形成及术前视觉模拟评分法(VAS)评分,通过单因素进行χ~2检验与Spearman相关性检验及多因素logistic回归分析膝关节骨关节炎手术治疗效果的影响因素。结果 80例膝关节骨关节患者手术治疗后手术效果低下8例,占比10.00%;72例手术效果良好,占比90.00%;采用Spearman相关性分析显示,软骨损伤(r=0.775,P<0.001)、韧带损伤(r=0.775,P<0.001)、术后感染(r=0.480,P<0.001)及术后下肢深静脉血栓形成(r=0.592,P<0.001)与手术治疗呈正相关(r>0,P<0.05);经单因素及Logistic回归分析发现,软骨损伤(OR=2.023,95%置信区间=1.602~35.717)、韧带损伤(OR=2.335,95%置信区间=2.129~50.144)、术后感染(OR=3.164,95%置信区间=1.865~300.377)、术后下肢深静脉血栓形成(OR=3.045,95%置信区间=2.825~156.118)均是膝关节骨关节炎手术治疗效果的影响因素(OR>1,P<0.05)。结论 软骨损伤、韧带损伤、术后感染及术后下肢深静脉血栓形成是膝关节骨关节炎手术治疗效果的影响因素,故临床应提前制定针对性干预措施,改善患者预后。

【Abstract】 Objective To investigate the influencing factors on effect of surgical treatment of knee osteoarthritis, in order to formulate targeted intervention measures.Methods 80 patients with knee osteoarthritis admitted to our hospital from January 2019 to December 2019 were selected as the research subjects, all patients were treated with artificial joint replacement, after 3 months of follow-up, the American Knee Society(AKS)score was used to evaluate the comprehensive improvement of knee joint in the two groups.The baseline data of all patients were collected, including free body, body mass index(BMI),cartilage injury, ligament injury, meniscus injury, postoperative infection, postoperative deep venous thrombosis and preoperative visual analogue scale(VAS)score, the influencing factors on effect of surgical treatment of knee osteoarthritis were analyzed by univariate with square test, Spearman correlation test and multivariate logistic regression.Results Of 80 cases of patients with knee osteoarthritis after surgical treatment, 8 cases had poor surgical effect, accounting for 10.00%;72 cases had good surgical effect, accounting for 90.00%;Spearman correlation analysis was used and showed that cartilage injury(r=0.775,P<0.001),ligament injury(r=0.775,P<0.001),postoperative infection(r=0.480,P<0.001)and postoperative deep vein thrombosis(r=0.592,P<0.001)were positively correlated with surgical treatment(r>0,P<0.05);univariate and Logistic regression analysis showed that cartilage injury(OR=2.023,95% confidence interval=1.602~35.717),ligament injury(OR=2.335,95% confidence interval=2.129~50.144),postoperative infection(OR=3.164,95% confidence interval=1.865~300.377),postoperative deep vein thrombosis(OR=3.045,95% confidence interval=2.825~156.118)were the influencing factors of effect of surgical treatment of knee osteoarthritis(OR>1,P<0.05).Conclusion Cartilage injury, ligament injury, postoperative infection and postoperative deep vein thrombosis are the influencing factors on effect of surgical treatment of knee osteoarthritis, therefore, targeted intervention measures should be formulated in advance to improve the prognosis of patients.
