血浆sCD36水平对冠状动脉粥样硬化斑块稳定性的影响

【摘要】 目的 探讨血浆sCD36水平对冠状动脉粥样硬化斑块稳定性的影响。方法 选择2018年1月至2021年1月冠状动脉粥样硬化性心脏病患者127例,分为A、B、C 3组,通过酶联免疫吸附测定法检测血浆sCD36浓度,通过血管内超声检查冠状动脉粥样硬化斑块的组织特征,比较各组基本特征及动脉粥样硬化斑块特征,分析富含脂质斑块的独立预测因素。结果 体重指数、高血压、血脂异常、ACS患病率、血浆sCD36水平及硝酸甘油使用各组间差异有统计学意义(P<0.05)。C组总脂质体积增高,而纤维体积和致密纤维体积则降低(P<0.05)。sCD36水平与脂质体积显著相关(P<0.001),而与纤维体积呈负相关(P<0.001)。sCD36水平是独立与富含脂质斑块存在显著相关的因素(P<0.001)。结论 血浆sCD36在临床症状严重的患者中表达升高,并与斑块脂质体积显著相关。

【Abstract】 Objective To investigate the effect of plasma sCD36 levels on the stability of coronary atherosclerotic plaques. Methods A total of 127 patients with coronary heart disease admitted to Hangzhou Third People’s Hospital from January 2018 to January 2021 were selected and divided into three groups: A, B and C. The sCD36 in plasma samples was detected by enzyme-linked immunosorbent assay(ELISA). Concentrations were detected, and the tissue characteristics of coronary atherosclerotic plaques were examined by intravascular ultrasound to compare basic characteristics, atherosclerotic plaque characteristics, and independent predictors of lipid-rich plaques. Results Body mass index, hypertension, dyslipidemia, prevalence of ACS, plasma sCD36level and nitroglycerin use were significantly different among groups(P<0.05). The total lipid volume fraction was significantly higher in group C, while the fiber volume and dense fiber volume were significantly lower(P<0.05). sCD36 levels were significantly correlated with lipid volume percentage(P<0.001)and negatively correlated with fiber volume percentage(P<0.001). sCD36 levels were independently and significantly associated with the presence of lipidrich plaques(P<0.001). Conclusion Plasma sCD36 expression is elevated in patients with severe clinical symptoms, and is significantly correlated with the percentage of plaque lipid volume.
