【摘要】 目的 分析IgA肾病患者肾小球系膜区C3沉积强度与临床指标及病理特征的相关性。方法 选取河南科技大学第一附属医院肾脏内科2020年1月至2021年12月收治的80例IgA肾病患者为研究对象，依据肾小球系膜区C3沉积强度分为阴性组15例、弱阳性组29例、强阳性组36例。测定并比较三组临床指标(镜下血尿、24 h尿蛋白、肌酐、尿素氮、尿酸、肾小球滤过率)、病理特征(肾小球、肾小管-间质)积分差异，采用Spearman相关性分析法检验肾小球系膜区C3沉积强度与临床指标及病理特征的相关性。结果 三组间肌酐、尿素氮、尿酸、肾小球滤过率、病理特征积分比较，差异有统计学意义(P <0.05)，镜下血尿、24 h尿蛋白比较，差异无统计学意义(P> 0.05);Spearman相关性分析结果显示：肾小球系膜区C3沉积强度与镜下血尿、24 h尿蛋白无相关性，差异无统计学意义(P> 0.05)，与肌酐、尿素氮、尿酸、病理特征相关积分呈正相关性，差异有统计学意义(P <0.05)，与肾小球滤过率呈负相关性，差异有统计学意义(P <0.05)。结论 IgA肾病患者肾小球系膜区C3沉积强度与临床指标及病理特征具有重要的相关性，可用于病情评估。更多还原

【Abstract】 Objective To analyze the correlation between the intensity of C3 deposition in the mesangial region of the glomerulus and the clinical and pathological characteristics of patients with IgA nephropathy. Methods A total of 80 patients with IgA nephropathy who were admitted to the department of Nephrology, The First Affiliated Hospital of Henan University of Science and Technology from January 2020 to December 2021 were selected as the research objects. According to the strength of C3 deposition in glomerular mesangial area, 15 cases were negative group, 29 were weak positive group and 36 were strong positive group.The clinical indicators(microscopic hematuria, 24-h urine protein, creatinine, blood urea nitrogen, uric acid,glomerular filtration rate), and pathological features(glomerular, tubule-interstitial) scores were measured and compared among the three groups, and the correlation between C3 deposition intensity and clinical index and pathological features of glomerular mesangial region was examined by Spearman correlation analysis. Results There were statistically significant differences in creatinine, blood urea nitrogen, uric acid, glomerular filtration rate, and pathological feature scores among the three groups(P <0.05).There was no significant difference in the comparison of microscopic hematuria and 24-hour urine protein(P > 0.05); The results of Spearman correlation analysis showed that the intensity of C3 deposition in the mesangial area was not correlated with microscopic hematuria and 24-hour urine protein(P > 0.05), but was positively correlated with creatinine, blood urea nitrogen,uric acid, and pathological feature scores(P < 0.05), which was negatively correlated with glomerular filtration rate(P <0.05). Conclusion The intensity of C3 deposition in the mesangial area of patients with IgA nephropathy may have an important correlation with clinical indicators and pathological characteristics, which can be used for disease evaluation.更多还原
