非小细胞肺癌脑转移患者肺癌组织Nrf2基因和蛋白表达及对脑转移预测价值

【摘要】 目的 观察非小细胞肺癌(NSCLC)脑转移患者肺癌组织核转录因子红系2相关因子2(Nrf2)基因和蛋白表达情况及对肺癌脑转移的预测价值。方法 将2019年9月—2021年9月收治的NSCLC脑转移65例为脑转移组,将非脑转移65例作为非脑转移组。比较2组Nrf2 mRNA、Nrf2蛋白表达情况,分析Nrf2 mRNA对NSCLC患者发生脑转移的预测价值。采用多因素Logistic回归分析NSCLC患者发生脑转移的危险因素。结果 Nrf2 mRNA预测NSCLC患者发生脑转移的曲线下面积为0.829。年龄≥60岁、腺癌、T分期为T4期、N分期为N2~N3期、Nrf2 mRNA≥1.149、Nrf2蛋白阳性是NSCLC患者发生脑转移的危险因素(P<0.05,P<0.01)。结论 Nrf2 mRNA和蛋白表达在NSCLC患者脑转移时升高,且为发生脑转移的危险因素,并对预测脑转移有一定价值。

【Abstract】 Objective To observe the gene and protein expression of nuclear transcription factor erythroid 2-related factor 2(Nrf2) in lung cancer tissues from non-small cell lung cancer(NSCLC) patients with brain metastases and its predictive value for brain metastases from lung cancer. Methods In total, 65 cases of NSCLC with brain metastases who were treated from September 2019 to September 2021 were selected as the brain metastasis group, and 65 cases with non-brain metastasis were selected as the non-brain metastasis group. The expression of Nrf2 mRNA and Nrf2 protein were compared between the two groups, and the predictive value of Nrf2 mRNA for brain metastasis in NSCLC patients was analyzed. Multivariate Logistic regression analysis was used to analyze the risk factors of brain metastasis in NSCLC patients. Results The area under the ROC curve of Nrf2 mRNA for predicting brain metastasis in NSCLC patients was 0.829. Age ≥60 years, adenocarcinoma, T4 stage, N2-N3 stage, Nrf2 mRNA ≥1.149, and positive Nrf2 protein were risk factors for brain metastasis in NSCLC patients(P<0.05,P<0.01). Conclusion The mRNA and protein expression of Nrf2 is increased in NSCLC patients with brain metastases, which is a risk factor for brain metastases, and has a certain value in predicting brain metastasis.
