腹腔镜手术治疗对盆腔炎性包块患者近期疗效、免疫功能、炎性指标及术后复发的影响

【摘要】 目的 探讨腹腔镜手术、开腹手术治疗对盆腔炎性包块患者近期疗效、免疫功能、炎性指标及术后复发的影响。方法 选取2017年12月—2020年12月本院行手术治疗的89例盆腔炎性包块,据治疗方案差异分为观察组42例(腹腔镜手术治疗)和对照组47例(开腹手术治疗)。比较2组近期疗效,术前1 d及术后12、24 h免疫功能指标、炎性指标,并观察2组围术期指标及术后复发情况。结果 观察组总有效率显著高于对照组(P<0.01)。观察组术后12、24 h免疫球蛋白G、免疫球蛋白M、免疫球蛋白A、白细胞介素-18、CD4+、CD4+/CD8+水平均高于对照组,白细胞介素-10、CD8+水平低于对照组(P<0.05)。观察组手术时间、术后体温恢复时间、术后肛门排气时间、术后血象恢复时间及住院时间均短于对照组(P<0.01)。观察组术后6个月复发率低于对照组(P<0.05)。结论 腹腔镜手术治疗盆腔炎性包块患者效果较理想,对改善患者免疫功能、炎性指标及预防术后复发有重要作用。

【Abstract】 Objective To investigate effects of laparoscopic surgery and open surgery on short-term efficacy, immune function, inflammatory indexes and postoperative recurrence in treatment of patients with pelvic inflammatory mass. Methods A total of 89 patients with pelvic inflammatory mass treated between December 2017 and December 2020 were selected and divided into observation group(laparoscopic surgery, n=42) and control group(open surgery, n=47) according to different surgical schemes. In two groups, short-term efficacy, immune function indexes and inflammatory indexes of the two groups at 1 d before operation, and after operation for 12 and 24 h were compared, and the perioperative indexes and conditions of postoperative recurrence were observed. Results The total effective rate in observation group was significantly higher than that in control group(P<0.01). After operation for 12 and 24 h, in observation group, levels of immunoglobulin G, immunoglobulin M, immunoglobulin A, interleukin(IL)-18, CD4+ and CD4+/CD8+ were significantly higher, while levels of IL-10 and CD8+ were significantly lower than those in control group(P<0.05). Operative duration, recovery time of postoperative body temperature, postoperative anal exhaust time, postoperative hemogram recovery time and length of hospital stay in observation group were significantly shorter than those in control group(P<0.01). The recurrence rate after operation for 6 months in observation group was significantly lower than that in control group(P<0.05). Conclusion Laparoscopic surgery is effective in treatment of patients with pelvic inflammatory mass, which plays an important role in improving immune function and inflammatory indexes and preventing postoperative recurrence.
