新生儿化脓性脑膜炎患儿血清PCT及CGRP水平与预后的相关性

【摘要】 目的 探讨新生儿化脓性脑膜炎患儿血清降钙素原、降钙素基因相关肽水平与预后的相关性。方法 检测70例新生儿化脓性脑膜炎患儿血清降钙素原、降钙素基因相关肽水平,比较不同预后患儿基线资料及血清降钙素原、降钙素基因相关肽水平,分析血清降钙素原、降钙素基因相关肽水平对预后的影响及预测价值。结果 70例新生儿化脓性脑膜炎患儿预后不良发生率为18.6%。预后不良患儿血清降钙素原、降钙素基因相关肽水平显著高于预后良好患儿(P<0.01),不同预后患儿性别、胎龄、出生体质量、分娩方式、发病日龄比较差异无统计学意义(P>0.05)。回归分析显示,血清降钙素原、降钙素基因相关肽过表达是新生儿化脓性脑膜炎预后不良的影响因素(P<0.05或0.01)。受试者工作特征曲线显示,血清降钙素原、降钙素基因相关肽水平预测新生儿化脓性脑膜炎患儿预后不良风险的曲线下面积分别为0.810、0.808,有一定预测价值。结论 新生儿化脓性脑膜炎患儿血清降钙素原、降钙素基因相关肽水平较高,其过表达可能参与疾病进程,影响预后,早期检测血清降钙素原、降钙素基因相关肽水平可预测预后不良风险,可能对该病的早期干预及改善预后具有积极意义。

【Abstract】 Objective To investigate the correlation between serum procalcitonin(PCT) and calcitonin gene related peptide(CGRP) levels and prognosis in children with neonatal purulent meningitis. Methods The levels of serum PCT and CGRP in 70 children with neonatal suppurative purulent were detected. The baseline data and serum PCT and CGRP levels of children with different prognosis were compared, and the influence and predictive value of serum PCT and CGRP levels on prognosis were analyzed. Results The incidence of poor prognosis in 70 infants with neonatal purulent meningitis was 18.6%. The levels of serum PCT and CGRP in children with poor prognosis were significantly higher than those in children with good prognosis(P<0.01). There was no significant difference in gender, gestational age, birth weight, mode of delivery and age of onset among children with different prognosis(P>0.05). Regression analysis showed that the overexpression of serum PCT and CGRP was a factor affecting the poor prognosis of neonatal purulent meningitis(P<0.05 or 0.01). The receiver operating characteristic(ROC) showed that the areas under the curve of serum PCT and CGRP levels in predicting the risk of poor prognosis in children with neonatal purulent meningitis were 0.810 and 0.808 respectively, which had certain predictive value. Conclusions The levels of serum PCT and CGRP in children with neonatal purulent meningitis are high, and their overexpression may participate in the disease process and affect the prognosis. Early detection of serum PCT and CGRP levels can predict the risk of poor prognosis, which may be of positive significance for early intervention and improvement of prognosis.
