3D打印技术辅助Salter截骨治疗发育性髋关节发育不良

【摘要】 目的探讨3D打印技术辅助Salter截骨治疗发育性髋关节发育不良的临床疗效。方法将34例发育性髋关节发育不良患儿按手术方式不同分为观察组(行3D打印技术辅助Salter截骨术治疗)与对照组(行Salter截骨术治疗),每组17例。记录两组手术时间、术中出血量、术中透视次数及并发症发生情况,测量髋臼指数,采用Severin X线评级评价髋关节形态、Mackay评定标准评价髋关节功能。结果两组均获得随访,时间12~26个月。手术时间、术中出血量和术中透视次数观察组均明显短(少)于对照组(P <0.05)。两组均未发生内固定松动、断裂,关节脱位及股骨头无菌性坏死等并发症。末次随访时两组髋臼指数均较术前显著下降(P <0.05),组间比较差异无统计学意义(P> 0.05)。末次随访时两组髋关节形态及髋关节功能优良率比较差异均无统计学意义(P> 0.05)。结论 3D打印技术辅助Salter截骨治疗发育性髋关节发育不良,可缩短手术时间,减少术中出血量和术中透视次数,手术疗效确切。

【Abstract】 Objective To investigate the clinical efficacy of three-dimensional( 3 D) printing assisted Salter osteotomy in the treatment of developmental dysplasia of the hip( DDH). Methods Thirty-four children with DDH were divided into the observation group( 3 D printing technique assisted Salter osteotomy) and the control group( Salter osteotomy),according to different surgical methods,with 17 cases in each group. The operation time,intraoperative blood loss,intraoperative fluoroscopy times and the complications of the two groups were recorded,and the acetabular index was measured. The morphology and function of hip were evaluated by Severin X-ray grade and Mackay evaluation standard. Results Both groups were followed up for 12 ~ 26 months. The operation time,intraoperative blood loss and intraoperative fluoroscopy times in the observation group were significantly shorter( less) than those in the control group( P < 0. 05). There were no complications such as internal fixation loosening,fracture,joint dislocation and aseptic necrosis of femoral head in the two groups. At the last follow-up,the acetabular indexes of the two groups were significantly decreased than those before operation( P < 0. 05),there was no significant difference between the two groups( P > 0. 05). At the last follow-up,there were no significant differences in Severin X-ray grade and excellentgood rate of hip joint function between the two groups( P > 0. 05). Conclusions In the treatment of DDH,3 D printing technique assisted Salter osteotomy can shorten the operation time,reduce intraoperative blood loss and intraoperative fluoroscopy times,and the surgical effect is accurate.
