济源市一起致泻大肠埃希菌引起食源性疾病的实验室检测及溯源分析
Laboratory Detection and Traceability Analysis of a Food-borne Cisease Caused by Diarrheagenic EsCherichia Coli in Jiyuan
【摘要】 目的 分析2020年河南省济源市报告的一起食源性疾病暴发事件,查明事件原因,制定有效的食源性疾病预防控制措施。方法 采用现场流行病学方法对此次食源性疾病暴发事件开展病例调查;采集可疑剩余食品及患者呕吐物和粪便等样本,以传统方法进行食源性致病菌的分离鉴定,采用多重荧光PCR法对分离到的致病性大肠埃希菌进行毒力基因检测,并用脉冲场凝胶电泳(PFGE)对菌株聚类分析。结果 调查病例79例,罹患率25.08%(79/315),临床表现以腹痛、恶心和腹泻为主;采集标本22份(食品样本10份,患者标本12份),从五香牛肉、大刀耳片和3份粪便共计5份样本中检出致病性大肠埃希菌,检出率22.73%;分离菌株18株,经多重荧光PCR检测,均显示肠聚集性大肠埃希菌(EAEC)毒力基因阳性;所有菌株经PFGE分析显示同源性>85%。结论根据现场流行病学调查和实验室检测结果,可判断该食源性疾病是因食用被EAEC污染的食品引起;多重荧光PCR法和PFGE分型技术可作为致病性大肠埃希氏菌检测的重要参考依据。
【Abstract】 Objective This study aims to analyze an outbreak of food-borne disease reported in Jiyuan city,Henan Province in 2020,and to find out the causes of this outbreak and formulate effective measures for the prevention and control of food-borne diseases. Methods Field epidemiological method was used to investigate the outbreak of food-borne diseases. Samples of suspect surplus food and patients’ vomitus and faeces were collected for isolation and identification of food-borne pathogens by traditional methods. Multiple fluorescent PCR was utilized to detect the pathogenic Escherichia coli. Cluster analysis of strains was performed by pulsed field gel electrophoresis(PFGE). Results A total of 79 cases were investigated,with anattack rate of 25.08%(79/315). The main clinical manifestations were abdominal pain,nauseaand diarrhea. Twenty-two samples were collected,including 10 food samples and 12 patient samples. Pathogenic Escherichia coli was detected in 5 samples,and the detection rate was 22.73%. Eighteen strains were isolated and showed positive virulence genes of enteroaggregative Escherichia coli(EAEC). PFGE analysis showed that the homology of all strains was more than 85%. Conclusion Based on the results of field epidemiological investigation and laboratory testing,it can be concluded that the food-borne disease was caused by eating food contaminated with EAEC,and the multiplex PCR and PFGE typing can be used as important reference for the detection of pathogenic E. Coli.
