胆囊结石患者血清胆汁酸谱的检测与分析
Detection and analysis of serum bile acid spectrum in patients with gallstone
【摘要】 目的 探讨血清胆汁酸谱的测定在胆囊结石临床诊断及早期预防中的应用价值。方法 选取2019年9月至2020年6月在河北医科大学第一医院住院的48例胆囊结石患者作为胆囊结石组,将其中32例总胆汁酸正常者作为总胆汁酸正常组,另选取26例体检健康者作为对照组。采用超高效液相色谱-串联质谱(UPLC-MS/MS)检测各组胆汁酸谱,并比较各组血清15种胆汁酸亚组分水平。结果 15种胆汁酸亚组分中,胆囊结石组6种胆汁酸亚组分[甘氨胆酸(GCA)、甘氨鹅脱氧胆酸(GCDCA)、甘氨熊脱氧胆酸(GUDCA)、牛磺胆酸(TCA)、牛磺鹅脱氧胆酸(TCDCA)及牛磺熊脱氧胆酸(TUDCA)]水平高于对照组,差异均有统计学意义(P<0.05)。总胆汁酸正常组脱氧胆酸(DCA)、熊脱氧胆酸(UDCA)低于对照组,GUDCA、TCA高于对照组,差异均有统计学意义(P<0.05)。结论 GUDCA、TCA可能为胆囊结石临床诊断及早期预防的重要标志物。
【Abstract】 Objective To evaluate the application value of serum bile acid spectrum in clinical diagnosis and early prevention of gallstone.Methods A total of 48 patients with gallstone hospitalized in the First Hospital of Hebei Medical University from September 2019 to June 2020 were selected as the gallstone group, among which 32 patients with normal total bile acid were selected as the normal total bile acid group.At the same time, 26 healthy subjects were selected as the control group.Ultrahigh performance liquid chromatography-tandem mass spectrometry(UPLC-MS/MS) was used to detect the bile acid spectra of each group, and the levels of 15 bile acid subcomponents in serum of each group were compared.Results Among the 15 bile acid subcomponents, the contents of 6 bile acid subcomponents [glycocholic acid(GCA),glycochenodeoxycholic acid(GCDCA),glycoursodeoxycholic acid(GUDCA),taurocholate(TCA),tauroursodeoxycholic acid(TCDCA),and taurochondoxycholic acid(TUDCA)] in gallstone group were significantly higher than those in control group, and the differences were statistically significant(P<0.05).Deoxycholic acid(DCA) and ursodeoxycholic acid(UDCA) in normal total bile acid group were lower than those in control group, while GUDCA and TCA were statistically higher than those in control group(P<0.05).Conclusion GUDCA and TCA may be important markers for clinical diagnosis and early prevention of gallstone.
