【摘要】 目的：分析豆袋热敷联合针灸、穴位按摩在初产妇分娩镇痛效果及对产程影响。方法：选取本院2018年1月-2020年1月收治的待分娩初产妇400例，按随机数字表法简单分组为观察组和对照组各200例，两组均使用针刺镇痛及穴位按摩镇痛，观察组联合使用豆袋热敷，比较两组产程时间、镇痛效果、宫缩强度及新生儿情况，使用视觉模拟评分法(VAS)和舒适状况量表(GCQ)评估初产妇疼痛程度及舒适度。结果：观察组第一、第二及总产程时间均短于对照组，初产妇镇痛总有效率(93.0%)高于对照组(82.0%),镇痛30min后两组宫缩强度均提高且观察组(81.32±26.79mmHg)高于对照组(57.89±17.45mmHg),VAS评分观察组镇痛30min(5.27±1.04分)、镇痛60min(4.68±0.53分)以及镇痛120min(4.13±0.59分)均低于对照组(6.76±0.72分、6.15±0.48分、5.77±0.62分)(均P<0.05);GCQ评估，观察组心理精神(20.72±4.63分)和生理评分(21.21±4.75分)均高于对照组(17.71±4.38分、15.89±4.59分)(均P<0.05);新生儿1min Apgar评分观察组和对照组无差异(P>0.05)。结论：豆袋热敷联合针灸、穴位按摩有利于缩短初产妇产程，增强宫缩强度并可有效减轻分娩疼痛，提高初产妇分娩舒适度。更多还原

【Abstract】 Objective: To analyze the effect of hot compress of bean bag combined with acupuncture and acupoint massage for labor analgesia of primiparae, and to study its influence on the stage of labor. Methods: A total of 400 primiparae were selected in this study from January 2018 to January 2020. These primiparae were divided into two groups(200 cases in each group) by random number table method. The primiparae in control group received acupuncture and acupoint massage for analgesia, and the primiparae in observation group received acupuncture and acupoint massage combined with hot compress of bean bag for analgesia. The labor duration, analgesic effect, and uterine contraction intensity of the primiparae, and the neonatal conditions were compared between the two groups. Visual analogue scale(VAS) and the general comfort questionnaire(GCQ) were used to evaluate the pain degree and the comfort level of the primiparae in the two groups. Results: The time of the first stage of labor, the second stage of labor, or the total labor of the primiparae in the observation group were significantly shorter than that of the primiparae in the control group. The total effective rate of analgesia(93.0%) of the primiparae in the observation group was significantly higher than that(82.0%) of the primiparae in the control group, and the uterine contraction intensity of the primiparae in the two groups had increased after 30 min of analgesia, and which(81.32±26.79 mmHg) of the primiparae in the observation group was significantly higher than that(57.89±17.45 mmHg) of the primiparae in the control group. VAS scores of the primiparae in the observation group at analgesia 30 min(5.27±1.04 points), analgesia 60 min(4.68±0.53 points),and analgesia 120min(4.13±0.59points)of the primiparae in the observation group were significantly lower than those(6.76±0.72points,6.15±0.48points,and 5.77±0.62points)of the primiparae in the control group(all P<0.05).As for GCQ evaluation,the scores of psychophysiology(20.72±4.63points)and physiology(21.21±4.75points)of the primiparae in the observation group were significantly higher than those(17.71±4.38points and 15.89±4.59points)of the primiparae in the control group(P<0.05).There was no significant difference in 1min Apgar score of the newborns between the two groups(P>0.05).Conclusion:Hot compress of bean bag combined with acupuncture and acupoint massage for the primiparae during delivery is beneficial to shorten their stage of labor,enhance their uterine contraction intensity,effectively reduce their labor pain,and improve their comfort level,which is worthy of application and promotion in clinical practice.更多还原
