【摘要】 目的 探讨采用双LVIS支架套叠辅助弹簧圈栓塞治疗对颅内血泡样动脉瘤(BBA)患者疗效及围术期并发症的影响。方法 行支架辅助弹簧圈栓塞治疗的BBA患者41例，将其分成非双LVIS支架组25例、双LVIS支架组16例，对两组手术效果、围术期并发症发生情况、血流动力学指标血清可溶性细胞间黏附分子-1(SICAM-1)、中枢神经特异性蛋白(S100β)水平及术后3、6个月时的复发率进行比较。结果 双LVIS支架组术后SICAM-1、S100β水平均较非双LVIS支架组降低(P<0.05)。双LVIS支架组术后3个月复发率较非双LVIS支架组降低(P<0.05),而术后6个月差异无显著性(P>0.05)。双LVIS支架组术后红细胞聚集指数均较非双LVIS支架组降低(P<0.05),而红细胞变形指数较非双LVIS支架组升高(P<0.05)。结论 采用双LVIS支架套叠辅助弹簧圈栓塞治疗BBA,效果好且并发症少，能显著降低SICAM-1、S100β水平，改善血流动力学，降低术后3个月复发率，应用价值高。更多还原

【Abstract】 Aim To investigate the effect of double LVIs stent intussusception assisted coil embolization on the efficacy and perioperative complications of patients with intracranial bullous aneurysms(BBA). Methods patients with BBA underwent stent assisted coil embolization were selected and divided into two groups(according to different treatment methods): non double LVIs stent group(25 cases) and double LVIs stent group(16 cases). The operative effects, perioperative complications and hemodynamic indexes of the two groups were compared, Serum soluble intercellular adhesion molecule-1(sICAM-1) and central nervous system specific protein(S100) were compared between the twogroups β) Level and recurrence rate at 3 and 6 months after operation. Results There was no significant difference inthe immediate complete embolism rate and perioperative complication rate between the two groups(P>0.05). double LVIs stent group postoperative sICAM-1, S100 β The levels were lower than non double LVIs stent group(P<0.05). The recurrence rate of double LVIs stent group at 3 months after operation(6.25%) was lower than non double LVIs stent group(36.00%)(P<0.05), but the difference was not significant at 6 months after operation(P>0.05). The erythrocyte aggregation index of double LVIs stent group was lower than non double LVIs stent group(P<0.05), while the erythrocyte deformation index was higher than that of non double LVIs stent group(P<0.05). Conclusion For BBA patients, double LVIs stent intussusception assisted coil embolization is effective and has fewer complications, which can significantly reduce sICAM-1 and S100 β Level, improve hemorheology, and help to reduce the recurrence rate of patients at 3 months after operation.
