替罗非班冠状动脉注射给药预处理联合PCI术治疗AMI患者的回顾性研究
A Retrospective Study of Tirofiban Coronary Injection Preconditioning Combined with PCI in the Treatment of AMI Patients
【摘要】 目的 探讨替罗非班冠状动脉注射给药预处理联合经皮冠状动脉介入术(PCI)治疗急性心肌梗死(AMI)患者的疗效。方法 回顾性收集我院2020年2月至2021年10月60例AMI患者的临床资料,均行PCI术治疗,对照组采用阿司匹林联合氯吡格雷治疗,研究组在对照组基础上,术前给予替罗非班冠状动脉注射。比较两组PCI后心肌组织再灌注情况、不良反应及治疗前、治疗后1周心功能指标[左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)]、血清白介素-17(IL-17)、可溶性凝集素样氧化型低密度脂蛋白受体(s LOX)-1水平。结果 PCI后,,与对照组sum STRI>50%、TIMI 2~3级比率比较,观察组优于对照组(P<0.05);治疗后1周,与对照组LVEF、LVEDD、LVESD比较,观察组LVEF偏低,LVEDD、LVESD偏高(P<0.05);治疗1周后,与对照组血清IL-17、s LOX-1水平比较,观察组上述指标水平偏高(P<0.05);治疗1周后,两组不良反应发生率组间比较差异无统计学意义(P>0.05)。结论 替罗非班冠状动脉注射给药预处理联合PCI治疗AMI患者,可改善心肌组织再灌注情况及心功能,而血清IL-17、sLOX-1水平降低可能为其重要机制。
【Abstract】 Objective To investigate the efficacy of tirofiban preconditioning by coronary injection combined with percutaneous coronary intervention(PCI) in the treatment of patients with acute myocardial infarction(AMI). Methods The clinical data of 60 patients with AMI in our hospital from February 2020 to October 2021 were retrospectively collected, all of them were treated with PCI, the control group was treated with aspirin combined with clopidogrel, and the study group was given preoperative administration of Coronary injection of tirofiban. The myocardial tissue reperfusion, adverse reactions, and cardiac function indexes(left ventricular ejection fraction(LVEF), left ventricular end-diastolic diameter(LVEDD), left ventricular end-systolic diameter(LVESD) before treatment and one week after treatment were compared between the two groups after PCI)], serum interleukin-17(IL-17), soluble lectin-like oxidized low-density lipoprotein receptor(s LOX)-1 levels. Results After PCI, compared with the control group, the sum STRI > 50%, TIMI grade 2-3 ratio, observation group was better than the control group(P<0.05). One week after treatment, LVEF, LVEDD and LVESD in the observation group were lower and LVEDD and LVESD were higher than those in the control group(P<0.05). After 1 week of treatment, the levels of serum IL-17 and s LOX-1 in the observation group were higher than those in the control group(P<0.05). After 1 week of treatment, there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion Tirofiban coronary injection pretreatment combined with PCI in the treatment of AMI patients can improve myocardial tissue reperfusion and cardiac function, and the decrease of serum IL-17 and s LOX-1 levels may be an important mechanism.
