【摘要】 排卵障碍是多囊卵巢综合征（polycystic ovarian syndrome, PCOS）患者不孕的主要原因。笔者通过总结文献发现针刺在治疗PCOS不孕方面显现了一定的疗效和优势。本文将从6个方面分别阐述针刺在PCOS排卵障碍性不孕中的作用，针刺可以调节下丘脑—垂体—卵巢轴恢复新的动态平衡，纠正激素紊乱，改善卵巢功能；针刺可以改善血流微循环，促进炎症吸收消散，改善慢性炎症状态，降低对卵泡发育的抑制；针刺可以调控PI3K/Akt/m TOR自噬信号通路而降低卵巢颗粒细胞自噬水平，减少卵泡闭锁，改善卵巢多囊样改变；针刺可以影响Kisspeptin信号途径，降低患者Kisspeptin表达水平，促进卵泡发育成熟；针刺可以平衡调控卵巢中糖脂代谢紊乱，恢复卵巢内部环境，使细胞正常增殖分化，利于卵泡发育；针刺可以通过调节子宫内膜组织胞饮突、整合素αVβ3、雌孕激素受体、同源盒基因A10等小分子改善子宫内膜容受性提高受孕率，以探讨可能的作用机制和途径，以期为临床治疗提供理论参考。文章还就目前针刺治疗本病的作用机制提出了一些思考，期望后期的研究能够不断深入，以明确针刺治疗PCOS排卵障碍性不孕的相关作用机制。更多还原

【Abstract】 Ovulation disorder is the main cause of infertility in patients with polycystic ovary syndrome（PCOS）. By summarizing the literature, the author found that acupuncture shows certain efficacies and advantages in the treatment of PCOS infertility. This article will explain the role of acupuncture in PCOS ovulatory disorder infertility from six aspects: acupuncture can regulate the hypothalamus-pituitary-ovary axis to restore a new dynamic balance, correct hormone disorders and improve ovarian function; acupuncture can improve blood flow microcirculation, promote the absorption and dissipation of inflammation, improve chronic inflammation and reduce the inhibition of follicular development. Acupuncture can regulate PI3K/Akt/mTOR autophagy signal pathway and reduce the autophagy level of ovarian granulosa cells, reduce follicular atresia and improve ovarian polycystic;acupuncture can affect Kisspeptin signal pathway, reduce the expression level of Kisspeptin in patients,and promote follicular development and maturation; acupuncture can balance and regulate the disorder ofglucose and lipid metabolism in the ovary, restore the internal environment of the ovary, make cellsproliferate and differentiate normally, which is conducive to follicular development. Acupuncture canimprove the endometrial receptivity and increase the pregnancy rate by regulating the cellular process ofendometrium, integrin αVβ3, estrogen and progesterone receptor, homeobox gene A10 and other smallmolecules, in order to explore the possible mechanisms and pathways, and to provide theoreticalreferences for clinical treatment. This article also puts forward some thoughts on the current mechanismsof acupuncture treatment of PCOS, hoping that the later research should be further studied in order toclarify the related mechanisms of acupuncture in the treatment of infertility caused by ovulation disorder inPCOS.更多还原
