黄芪甲苷治疗肾病的药理作用研究进展

【摘要】 黄芪甲苷是源于蒙古黄芪Astragalus membranaceus(Fisch.)Bge. var. mongholicus(Bge.)Hsiao的三萜苷类成分,具备阻断细胞凋亡、抗炎症反应、抑制氧化应激、减轻肾纤维化、改善贫血、阻滞肿瘤发展等功效,在临床肾病应用方面有着良好的潜力。于调控细胞凋亡及蛋白层面,黄芪甲苷可经改善线粒体损伤、抑制内质网应激、维护裂孔蛋白稳定以阻滞足细胞凋亡;于抗炎性因子及炎症小体方面,黄芪甲苷把控上下级信号通路,束缚炎性因子和炎症小体的释放;于清除氧化自由基层面,黄芪甲苷通过减弱氧化反应、增强抗氧化效应来避免自由基积累,且能消弭内外毒素诱导的急慢性肾损伤;于抑制肾组织纤维化方面,黄芪甲苷降低纤维化因子的过表达,扭转上皮—间充质转化(epithelial to mesenchymal transition,EMT)以推迟肾纤维化进程。本文旨在探究黄芪甲苷治疗肾脏疾病药理作用的最新进展,为进一步实验和临床研究提供参考。

【Abstract】 Astragaloside IV is a triterpene glycoside component derived from Astragalus membranaceus, which has good potentials in clinical nephropathy applications due to the diversified functions. In general, Astragaloside IV has the effects of blocking apoptosis, anti-inflammation, antioxidation, alleviating renal interstitial fibrosis, improving anemia, retarding the progression of cancer and so on. In terms of regulating apoptosis proteins and pathways, Astragaloside IV is able to prevent podocyte apoptosis by improving mitochondrial damage, inhibiting endoplasmic reticulum stress, equilibrating calcium homeostasis, and maintaining the stability of slit protein. There is no need to elaborate that Astragaloside IV also play a considerable role on restraining renal epithelical cells and mesangial cells apoptosis. In the aspect of resisting inflammatory factors and inflammasome, Astragaloside IV control the upper and lower signaling pathway, restrict the release of inflammatory factors and inflammasomes, adjust the micro-inflammatory state of body, protect the vascular endothelial cell barrier. In the matter of scavenging oxidative free radicals, Astragaloside IV has the capability of avoiding the accumulation of free radicals by weakening the oxidation reaction and enhancing the antioxidant effects, as well as eliminating the acute and chronic kidney damages induced by exotoxin or endotoxin. In terms of inhibiting kidney fibrosis, Astragaloside IV realizes the goals of delaying the process of renal fibrosis by means of reducing the overexpression of fibrotic factors, intervening extracellular matrix deposition, and reversing EMT. This article aims to explore the latest developments in the pharmacological effects of Astragaloside IV in thetreatment of kidney diseases, and provide references for further experiments and clinical investigations.
