【摘要】 目的：观察补肾活血方治疗肾虚血瘀型老年H型高血压的临床疗效。方法：将126例肾虚血瘀型老年H型高血压患者按照随机数字表法分为对照组和治疗组，每组各63例。对照组口服马来酸依那普利叶酸片治疗，治疗组在对照组治疗的基础上加用补肾活血方。比较两组患者的临床疗效及治疗前后中医证候积分、同型半胱氨酸(homocysteine, HCY)、内皮素(endothelin, ET)、一氧化氮(nitric oxide, NO)、超敏C反应蛋白(hypersensitive C-reactive protein, hs-CRP)、动态血压、心脏功能指标[左心室射血分数(left ventricular ejection fraction, LVEF)、每搏输出量(stroke volume, SV)及6分钟步行距离(6-minute walk distance, 6MWD)]变化情况。结果：两组患者治疗后动态血压低于本组治疗前，且治疗后治疗组低于对照组，差异具有统计学意义(P<0.05)。治疗组有效率为95.24%,对照组有效率为79.37%,两组患者有效率比较，差异具有统计学意义(P<0.05)。两组患者治疗后HCY、ET、hs-CRP水平低于本组治疗前，且治疗后治疗组低于对照组；两组患者治疗后NO水平高于本组治疗前，且治疗后治疗组高于对照组，差异均有统计学意义(P<0.05)。两组患者治疗后SV、LVEF、6MWD高于本组治疗前，且治疗后治疗组高于对照组，差异均有统计学意义(P<0.05)。结论：补肾活血方治疗肾虚血瘀型老年H型高血压，可降低患者血压及炎性因子水平，明显改善血管内皮功能及心脏功能。更多还原

【Abstract】 Objective: To observe the clinical efficacy of Kidney-Nourishing and Blood-Activating Decoction on elderly H-type hypertension with kidney deficiency and blood stasis syndrome.Methods: A total of 126 elderly patients with H-type hypertension with kidney deficiency and blood stasis syndrome were randomly divided into the control group and the treatment group, with 63 cases in each group.The control group was treated with Enalapril Maleate Folic Acid Tablets, while the treatment group was treated with Kidney-Nourishing and Blood-Activating Decoction on the basis of the treatment of the control group.The clinical efficacy, the changes of TCM syndrome score, homocysteine(Hcy),endothelin(ET),nitric oxide(NO),hypersensitive C-reactive protein(hs-CRP),ambulatory blood pressure, cardiac function indexes [including left ventricular ejection fraction(LVEF),stroke volume(SV) and 6-minute walk distance(6 MWD)] before and after treatment were compared between two groups.Results: The ambulatory blood pressure of the two groups was lower than that of the same group before treatment, and the ambulatory blood pressure of the treatment group was lower than that of the control group, and the difference was statistically significant(P<0.05).The effective rate was 95.24% in the treatment group and 79.37% in the control group.There was significant difference between the two groups(P<0.05).After treatment, the levels of Hcy, ET and hs-CRP in the two groups were lower than those of the same group before treatment, and the above indexes in the treatment group were lower than those in the control group; The level of NO in the two groups after treatment was higher than that of the same group before treatment, and the level of NO in the treatment group after treatment was higher than that in the control group, and all the differences were statistically significant(P<0.05).The SV,LVEF and 6 MWD of the two groups after treatment were higher than those of the same group before treatment, and the above indexes of the treatment group were higher than those of the control group, and all the differences were statistically significant(P<0.05).Conclusion: Kidney-Nourishing and Blood-Activating Decoction in the treatment of elderly H-type hypertension with kidney deficiency and blood stasis syndrome can reduce the level of blood pressure and inflammatory factors, and significantly improve vascular endothelial function and cardiac function.更多还原
