【摘要】 目的：探究关节镜病灶清理术对老年膝关节退行性骨关节炎(KDOA)患者关节功能、疼痛程度的影响。方法：回顾性分析2015年3月～2018年3月在本院接受关节镜病灶清理术治疗的82例老年KDOA患者的临床资料，分别采用Lequesne指数及视觉疼痛模拟评分(VAS)评估各患者术前、术后1个月、3个月、6个月及术后1年膝关节功能及疼痛程度，通过Logistic回归分析对影响手术疗效的相关因素进行分析。结果：82例患者中67例患者手术成功，成功率为81.71%。术后1个月、3个月、6个月及术后1年患者VAS评分及Lequesne指数均明显低于术前。手术成功及手术失败患者年龄、体质量(BMI)、关节面夹角及关节内病损数目存在明显差异；BMI>25kg/m~2及关节面夹角≦7°是导致老年KDOA患者手术失败的独立危险因素。结论：关节镜病灶清理术治疗可明显改善老年KDOA患者关节功能并降低患者关节疼痛程度，但其疗效受到患者BMI及关节面夹角的影响。更多还原

【Abstract】 Objective To investigate the effects of arthroscopic debridement on joint function and pain degree of elderly patients with knee degenerative osteoarthritis (KDOA).Methods The clinical data of 82 elderly KDOA patients who underwent arthroscopic debridement in the hospital from March 2015 to March 2018 were retrospectively analyzed.The Lequesne index and visual analogue scale (VAS) were performed to evaluate knee joint function and pain degree before surgery and at 1 month,3 months,6 months and 1 year after surgery.Logistic regression analysis was performed to analyze related factors influencing surgical curative effect.Results Of the 82 patients,there were 67 cases undergoing successful surgery,with success rate of 81.71%.At 1 month,3 months,6 months and 1 year after surgery,VAS score and Lequesne index were significantly lower than those before surgery.There were significant differences in age,body mass index (BMI),inclined angle of articular surface and number of intra-articular lesions between among patients with successful and failed surgery.BMI greater than 25kg/m~2 and inclined angle of articular surface not greater than 7° were independent risk factors which lead to failed surgery in elderly KDOA patients.Conclusion Arthroscopic debridement can significantly improve joint function of elderly KDOA patients and reduce joint pain degree.However,its curative effect is affected by BMI and inclined angle of articular surface.更多还原
