【摘要】 目的：探究尿毒症患者应用血液灌流联合碳酸镧治疗对皮肤瘙痒疗效及钙磷代谢的影响。方法：选取2021年1月～2022年1月期间本院收治的81例尿毒症性皮肤瘙痒(UP)患者，根据随机数字表法分为两组，观察组40例，对照组41例。对照组采用常规血液透析(HD)进行治疗，观察组采用HD联合血液灌流(HP)并辅助碳酸镧治疗。治疗8周后，评估两组患者的疗效，比较治疗前、治疗8周后的瘙痒改善情况(5-D瘙痒评分)，血清钙、磷代谢水平，肾功能指标[尿素氮(BUN)、血肌酐(Scr)]及皮肤病生活质量指数量表(DQOLS量表)，比较两种治疗方案的安全性。结果：观察组治疗有效率明显高于对照组；治疗后，两组UP患者5-D瘙痒评分降低，观察组低于对照组；治疗后，两组UP患者血清钙水平升高，血磷水平降低，观察组血清钙、磷水平均优于对照组；治疗后，两组UP患者BUN、Scr水平均降低，观察组BUN、Scr水平低于对照组；治疗后，两组UP患者DQOLS评分升高，观察组DQOLS评分高于对照组；两组患者不良反应发生率比较，差异均无统计学意义。结论：血液灌流联合碳酸镧能够有效提高UP患者的疗效，改善瘙痒情况及钙磷代谢水平，同时提高患者肾功能及生活质量，安全有效，值得广泛应用。更多还原

【Abstract】 Objective To explore the effect of hemoperfusion combined with lanthanum carbonate on skin pruritus and calcium and phosphorus metabolism in patients with uremia.Methods A total of 81 patients with uremic pruritus (UP) admitted to our hospital from January 2021 to January 2022 were selected and divided into two groups according to the random number table method,with 40 cases in the observation group and 41 cases in the control group.The observation group was treated with HD combined with hemoperfusion (HP) and lanthanum carbonate.After 8 weeks of treatment,the curative effect of the two groups of patients was evaluated,and the improvement of pruritus (5-D pruritus score),serum calcium and phosphorus metabolism levels,and renal function indexes [Burea Nitrogen (BUN)]before and after 8 weeks of treatment were compared,serum creatinine (Scr)]and dermatological quality of life index scale (DQOLS scale),compare the safety of the two treatment options.Results The effective rate of treatment in the observation group was higher than the control group.After treatment,the 5-D pruritus score of UP patients in two groups decreased,and the observation group was lower than the control group.After treatment,the serum calcium level of UP patients in the two groups increased,and the serum phosphorus level decreased.The serum calcium and phosphorus levels in the observation group were better than those in the control group.After treatment,the levels of BUN and Scr in UP patients in two groups were decreased,and the observation group was lower than the control group.The incidence of adverse reactions in the observation group and the control group was no significant difference.Conclusion Blood perfusion combined with lanthanum carbonate can effectively improve the curative effect of patients with UP,improve itching and calcium and phosphorus metabolism.At the same time,it is safe and effective to improve the renal function and quality of life of patients,and is worthy of wide application.更多还原
