埃克替尼对比吉非替尼治疗晚期非小细胞肺癌患者的有效性和安全性分析
Efficacy and Safety of Icotinib Versus Gefitinib in the Treatment of Patients with Advanced Non-small Cell Lung Cancer: A Meta-analysis
【摘要】 目的 系统评价埃克替尼对比吉非替尼治疗晚期非小细胞肺癌(non-small cell lung cancer, NSCLC)患者的有效性和安全性,旨在为临床合理用药提供循证依据。方法 计算机检索PubMed、荷兰医学文摘(Excerpta Medica Database, EMBASE)数据库、考克兰图书馆(the Cochrane Library)、中国知网(China national knowledge infrastructure, CNKI)、万方数据库,收集埃克替尼(试验组)对比吉非替尼(对照组)治疗NSCLC患者的随机对照试验(randomized controlled trial, RCT)。检索时限均为从建库至2021-06-27日。由两名研究者按照纳入和排除标准独立完成筛选文献,提取数据和质量评价。采用RevMan 5.4软件进行Meta分析。结果 共纳入17项RCT,合计1591例患者。Meta分析结果显示,埃克替尼治疗晚期NSCLC的客观缓解率(objective response rate, ORR)、疾病控制率(disease control rate, DCR)、无进展生存率(progression-free survival, PFS)和总生存率(overall survival, OS)与吉非替尼相比,未见统计学差异(P>0.05)。但埃克替尼组皮疹、腹泻、肝功能异常发生率显著低于吉非替尼组,差异具有统计学意义(P<0.05)。结论 埃克替尼治疗晚期NSCLC的疗效与吉非替尼相当,安全性更好。
【Abstract】 Objective To systematically evaluate the efficacy and safety of icotinib versus gefitinib in the treatment of patients with advanced non-small cell lung cancer(NSCLC), and to provide evidence-based basis for clinical rational drug use. Methods PubMed, Excerpta Medica Database(EMBASE), the Cochrane Library, China national knowledge infrastructure(CNKI) and Wanfang were searched by computer, and the randomized controlled trial(RCT) of icotinib(experimental group) versus gefitinib(control group) in the treatment of NSCLC patients were collected. The searching time was from the establishment of the database to June 27, 2021. According to the inclusion and exclusion criteria, the screening of literature, extraction of data and quality evaluation were independently completed by two researchers. Meta-analysis was carried out by RevMan 5.4 software. Results A total of 17 RCTs with 1,591 patients were included in this study. Meta analysis showed that there was no significant difference in objective response rate(ORR), disease control rate(DCR), progression-free survival(PFS) and overall survival(OS) between icotinib and gefitinib in advanced NSCLC(P>0.05). However, the incidence of rash, diarrhea and abnormal liver function in the icotinib group was significantly lower than that in the gefitinib group(P<0.05). Conclusion The efficacy of icotinib in the treatment of advanced NSCLC is similar to that of gefitinib, but has better sucurity.
