呼吸道感染患儿EBV-DNA载量及与肝肾功能损伤的关系

【摘要】 目的 探讨呼吸道感染患儿爱泼斯坦-巴尔病毒(EBV)-脱氧核糖核酸(DNA)载量表达及其与肝肾功能损伤的关系研究。方法 选取2016年1月至2020年6月门诊及病房住院治疗经临床确诊的呼吸道感染儿童175例为研究对象,依据EBV-DNA载量水平分为阳性组(n=117)和阴性组(n=58),比较不同性别、年龄的呼吸道感染患儿EBV-DNA阳性率,根据EBV-DNA载量将阳性组患儿分为EBV-DNA高载量组(n=21)、中载量组(n=35)和低载量组(n=61),比较不同EBV-DNA载量组患儿肝肾功能异常比例及血清丙氨酸转氨酶(ALT)、天冬氨酸氨基转移酶(AST)、胱抑素C(Cystatin C,CysC)、β2微球蛋白(β2-microglobulin,β2-MG)水平,Pearson相关性分析EBV-DNA载量与ALT、AST、CysC、β2-MG的关系。结果 不同性别患儿间EBV-DNA阳性率差异无统计学意义(P>0.05),1~3岁、3~6岁年龄段儿童中EBV-DNA阳性率最高,不同年龄间患儿EBV-DNA阳性率差异有统计学意义(P<0.05);EBV-DNA低载量组、中载量组、高载量组的肝功能异常比例为19.67%、48.57%、66.67%,肾功能异常比例为13.11%、42.86%、52.38%,不同EBV-DNA载量患儿肝肾功能异常比例相比,差异有统计学意义(P<0.05);血清ALT、AST、CysC、β2-MG水平随EBV-DNA载量升高而升高(P<0.05),EBV-DNA载量与ALT、AST、CysC、β2-MG水平呈正相关性(P<0.05)。结论 呼吸道感染患儿EBV-DNA载量升高会增加发生肝肾功能损伤的风险,且在1~6岁儿童EBV-DNA阳性率较高,检测EBV-DNA载量有助于呼吸道感染患儿肝肾功能的损伤的评估。

【Abstract】 Objective To investigate the expression of Epstein-Barr virus deoxyribonucleic acid(EBV-DNA) load in children with respiratory tract infection and its correlation with hepatic and kidney damage of patients.Methods A total of 175 children with respiratory tract infection who were treated in our hospital from January 2016 to June 2019 were enrolled in the study. According to the EBV-DNA load levels, these patients were divided into positive group(n=117) and negative group(n=58).The EBV-DNA positive rates were observed and compared between the two groups.Moreover the patients in positive group were redivided into EBV-DNA high-load group(n=21),medium-load group(n=35) and low-load group(n=61).The proportion of abnormal liver and kidney function, and the serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),Cystatin C(CysC) and carrier 2 microglobulin(carrier 2-MG) were observed and compared among the three groups. Pearson correlation analysis was performed to analyze the correlation between the EBV-DNA load and ALT,AST,CysC,β2-MG.Results There was no significant difference in EBV-DNA positive rate between male patients and female patients(P>0.05),and EBV-DNA positive rate was the highest in 1~3 year group and 3~6 year group, moreover, there was significant difference in EBV-DNA positive rate in different age groups(P<0.05).The proportion of liver function abnormality in low-load, medium-load, high-load groups were 19.670%,48.570%,66.670%,respectively, and the proportion of renal function abnormality were 13.110%,42.860% and 52.380%,respectively, there were significant differences(P<0.05).In addition the serum levels of ALT,AST,CysC and β2-MG were increased with the increase of EBV-DNA load(P<0.05),which were positively correlated with EBV-DNA load(P<0.05).Conclusion The increase of EBV-DNA load in children with respiratory tract infection can increase the risk of liver and kidney function damage, and EBV-DNA positive rate is higher in children aged 1~6 years.To detect EBV-DNA load is helpful for assessing the liver and kidney function damage of patients.
