【摘要】 目的 探讨应用经外周静脉置入中心静脉导管(peripherally inserted central catheter, PICC)和中心静脉导管(central venous catheter, CVC)两种不同输液途径对恶性梗阻性黄疸行腹腔镜胰十二指肠切除(LPD)患者的影响。方法 采用随机数字表法将2019年1月至2020年6月恶性梗阻性黄疸拟行LPD的患者120例随机分为观察组和对照组，每组60例。对照组于手术时行CVC置管，观察组于术前1周内行PICC置管，比较2组患者导管留置时间、非计划性拔管情况、不良反应和患者自感舒适度的情况。结果 观察组患者导管留置时间、非计划性拔管情况、不良反应和舒适度情况明显优于对照组，差异有统计学意义(P<0.05)。结论 恶性梗阻性黄疸行LPD患者应用PICC组比CVC组导管留置时间、非计划性拔管、其他不良反应发生情况和患者舒适度方面处于明显优势地位，其次患者需多次住院输注化疗药物，避免了多次反复穿刺带来的痛苦，从而提高了患者舒适度和生活质量。更多还原

【Abstract】 Objective To investigate the therapeutic effects of peripherally inserted central catheter(PICC) and central venous catheter(CVC) for patients with obstructive jaundice undergoing laparoscopic pancreaticoduodenectomy(LPD).Methods A total of 120 patients with obstructive jaundice who underwent LPD were randomly divided into control group and observation group, with 60 cases in each group.The patients in control group received CVC catheterization during surgery, however, those in observation group received PICC within one week before surgery.The catheter indwelling time, unplanned extubation status, adverse reactions, patient’s comfort degree were observed and compared between the two groups.Results The catheter indwelling time, unplanned extubation status, adverse reactions, and comfort degree of patients in observation group were significantly better than those in control group(P<0.05).Conclusion For the patients with obstructive jaundice undergoing LPD,the PICC is superior to CVC in the aspects of catheter retention time, unplanned extubation status, adverse reactions comfort degree and life quality of patients.更多还原
