新型冠状病毒肺炎患者康复期肺功能及胸部CT随访
Follow-up of pulmonary function and chest CT in patients with COVID-19 at rehabilitative period
【摘要】 目的 通过分析新型冠状病毒肺炎患者康复期肺功能及胸部CT随访结果,为指导新型冠状病毒肺炎患者康复治疗提供依据。方法 以2021年1月3日至3月2日于邢台市第二医院救治并治愈的新型冠状病毒肺炎患者80例为研究对象,共收集患者出院后3~4个月的胸部CT结果67例,患者出院后3~4个月肺功能结果59例,根据在院时疾病严重程度分为轻型组、普通型组、重型组(含重型及危重型),并对随访结果进行分析。结果 最终随访患者中重型组平均年龄>其他2组(P=0.003),其男性占比较高(80.0%vs 20.0%);伴有基础疾病比例最高为高血压,重型组伴有基础疾病比例较非重型组高但除冠心病(P=0.013)外,余差异均无统计学意义(P>0.05);出院患者3~4个月随访中患者肺通气功能正常89.8%(53/59),患者弥散功能正常83.1%(49/59),重型组患者肺通气功能及弥散功能异常的患者占比较高;患者胸部CT未遗留明显异常61.2%,其余患者胸部CT最常见的异常影像学表现为条索影及磨玻璃影。结论 大部分COVID-19康复期患者肺功能及胸部CT已恢复正常,说明肺部病变是可逆的,但仍有部分患者遗留肺功能及影像学异常,且与在院时疾病严重程度呈正相关。
【Abstract】 Objective To investigate the results of follow-up of pulmonary function and chest CT of patients with COVID-19 at rehabilitative period, so as to provide basis for guiding the rehabilitation of patients with new coronavirus pneumonia.Methods A total of 80 patients with COVID-19 who were treated in our hospital from January 3,2021 to March 2,2021 were enrolled in the study.And the chest CT results were collected in 67 patients at 3~4 months after discharge, and the pulmonary function examination results of 59 patients at 3-4 months after discharge were collected.According to the severity of the disease at admission, these patients were divided into mild type, common type, severe type, and the follow-up results were analyzed.Results The average age in severe case group was significantly higher than that in the other two groups, especially for male patients.The highest proportion of basic diseases was hypertension, which in severe case group was significantly higher than that in non-severe case group.After 3~4 m-follow up, 89.8% of patients had normal pulmonary ventilation function, and 83.1% of them had normal diffusion function.Of the severe patients, the patients with abnormal pulmonary ventilation function and diffusion function accounted for a relatively high proportion, and 59.7% of them left no obvious abnormalities on chest CT,and the most common abnormal imaging findings in chest CT in the rest patients were strip shadows and ground glass shadows.Conclusion The lung function and chest CT in most patients with COVID-19 have returned to normal at rehabilitative period, which suggests that the lung lesions are reversible, but some patients still have lung function and imaging abnormalities, which are positively correlated with the severity of the disease.
