光学相干断层扫描血管成像在慢性闭角型青光眼小梁切除术后患者视盘周围血流监测中的应用价值

【摘要】 目的 分析光学相干断层扫描血管成像(OCTA)在慢性闭角型青光眼小梁切除术后患者视盘周围血流观察中的应用。方法 收集2017年6月—2019年1月收治的慢性闭角型青光眼32例48眼的临床资料,所有受试者均接受小梁切除术,在术前、术后3个月应用OCTA分别测量视盘周围、鼻侧、颞侧血管密度,分析血管密度与眼压的相关性。结果 本组术后3个月眼压低于术前,视盘周围、鼻侧及颞侧血管密度高于术前(P<0.05)。术后血管密度增高与眼压降低有关(P<0.01)。结论 慢性闭角型青光眼高眼压引起的眼灌注损伤可通过降低眼压得到改善,应用OCTA对视盘周围血流监测有重要意义。

【Abstract】 Objective To analyze the application of optical coherence tomography angiography(OCTA) in the observation of peripapillary blood flow in patients with chronic angle-closure glaucoma(CACG) after trabeculectomy. Methods The clinical data of 48 eyes of 32 patients with CACG treated from June 2017 to January 2019 was collected. All subjects underwent trabeculectomy. The vascular densities of the optic disc area, at nasal side and temporal side were measured by OCTA before and at 3 months after operation, and the correlation between vascular density and intraocular pressure(IOP) was analyzed. Results After operation, the IOP was lower than that before operation, and the vascular densities of the optic disc area, at nasal side and temporal side was higher than that before operation(P<0.05). Increased vascular densities after operation were related to the decrease of IOP(P<0.01). Conclusion The ocular perfusion injury caused by glaucoma ocular hypertension in patients with CACG can be ameliorated by reducing IOP, and the application of OCTA is of great significance for monitoring the peripapillary blood flow.
